For your Unit 9 Assignment, you will write a reflection paper from the point of view of a human service professional in his or her last job in the field. You can choose to be a Chief Executive Officer, or supervisor, or any other job that interests you. The purpose of your paper is to provide advice for others in your agency who are just beginning their careers in human services. You will include specific information from your Unit 3 and unit 6Assignment about using effective coordination between agencies in the community for service delivery. You will also describe some of the best methods for providing direct service to clients using the information from your Unit 6 Assignment. You will describe the importance of advocating for the needs of the people in your local community (and for the agencies that serve them).

One thing I learned in Human services is confidentiality no matter who you are talking to you have to be careful of your surroundings, any documents you have need to be kept in a secure place when not being worked on.

Therefore a job of a service coordinator is to link clients to the other agencies both profit and nonprofit where they can receive help. Guidelines for confidentiality Confidentiality is the protection of client’s personal information including personally identifying information from other people. When the client shares their information, the worker should include that whatever shared is kept between them and the client. In addition, information shared by the client would not be shared to coworkers, family or friends without the consent of the client. Thus our organization believes that confidentiality is important to our clients. Also, the agency is compliant with HIPPA laws on privacy of client’s information as well as the rights of the client to their information (Bond & Mitchels, 2014).Therefore it must be upheld during the process of referral. The agency has a referral form which is filled by the case worker together with the client. However, the information to be filled has be consented by the client before writing it for referral. Therefore the client has to fill a consent form allowing the agency to link them to an agency that is relevant to their problem. The agency worker is responsible for linking the client to an institution relevant for their problem. Also before referral the client is informed is reason for the referral and the agency they would be COORDINATION OF SERVICE PLAN referred to. In addition, the referral form does not contain clients personal information which they would not want disclosed. Guidelines for making and evaluating referral To refer a client to another agency would mean that there are other services which may not be available in our agency but are available in another agency. Using the professional network formed, our organization has a list of agencies profit and nonprofit to help serve clients comprehensively. Therefore a client is received by a general case worker at the entry stage. The case worker is tasked with identifying the services required by the client after taking their case history. Once the needs of the client are filtered, the case worker may refer the client to another agency if their assessment on them concludes that a referral is necessary. The case worker links up with the other agency to find out whether they can refer the client to the agency. However, the case worker does not use personally identifying information in the process. Once the other agency accepts a referral then the client would fill a referral consent form. However, making a referral does not men closing the case of a client, the agency makes a follow up on the case to know how the client is fairing after the referral. Identifying barriers Referral may seem to be as easy as writing a simple referral form. However, there may be barriers to it. The barriers may include transportation of the client from our agency to another especially in cases where there are many referral clients who may need to go to different agencies. Also, some clients may not be comfortable being referred to 3 COORDINATION OF SERVICE PLAN certain agencies that are profit making due to financial difficulties. Therefore a case worker has to inform the client the type of agency before making a referral to enable them make a decision on whether they have the necessary finances or not. Timeline for referral A client in a crisis situation may not be suitable for holding up the case for a week. The worker staff attending to the client should be keen to identify the stage their client is in. In case the person has a crisis, then crisis intervention should be employed immediately before making a referral to another agency. An individual’s reaction to certain situation may be different from another and therefore the counselor may not put a specific timeframe that is generic to all clients. An individual client may take a week to open up and let the counselor know what needs to be addressed and by who (Tribe, & Morrissey, 2015). Similarly, some social problems may need immediate referral for immediate intervention. For instance a client in need of food would be referred immediately. Once their food problem is solved, the referring counselor would need to make a follow up with the client. Nevertheless, clients may be having an intervention schedule basing on their problem, their availability as well as avoiding an overlap on other services. Some may range from a visit on the second day after enrollment while others may be a week later. However, clients in crisis are served immediately. Guidelines for coordinating services for clients 4 COORDINATION OF SERVICE PLAN Some clients may have referrals for two or more services therefore it is necessary to have a coordination guideline tailor made for each client. For instance a client who came in in need of food stamps may be referred be enrolled for job training in our organization, counseling on financial management in another financial agency and another session for parenting in another (Nystul, 2015). After losing her job and subsequently being evicted from the public house, it is evident that things have gotten worse for her and her family. Although her new boyfriend offered them refuge and she have moved to his house with her six children, everything is still not right. First, they are living in a housing which according to the housing regulations is illegal. It is just a two bedroom house meaning that it is quite small to accommodate such a large family. Although Teresa is insisting that they are living well, it is not the case because she has nowhere better to stay and she and her family are just persevering with what they have. Despite the fact that now she has somewhere to call home the crisis seems to be haunting her even more. It is evident that everything is not right with her and her boyfriend. There seems to be a war in their home where Teresa and her boyfriend may be fighting on frequent occasions. It is explained by the scenario where when she is called, yells are heard and when a message is even left, it is not replied in an appropriate time. After two days of the incident she appears in the office with broken nose, black eye and a bandaged hand. When asked she seems to be covering for her boyfriend yet it is evident that he is a violent man. There fore his boyfriend taking care of the bills and even making a promise to the children is not all, there are big issues between the two and it would be a good idea to get Teresa a new house. Barriers to solving the problem The major barrier that would keep her from the resolution of the problem is her secretive nature. It seems that Teresa would take time to open up about some of the things that she is COORDINATION OF COUNSELING SERVICES 3 experiencing. For instance she is covering up the whole incident that led to her injuries. It is not clear whether she is doing so because she lacks another option of where to stay or whether she may be receiving threats from her boyfriend. It is therefore a significant issue as the only way out of the crisis is for her to open up everything about her experiences with will. Teresa needs to understand that there is the principle of confidentiality and thus any information she would provide would be secure (Tribe, &Morrissey, 2015). It is certain that Teresa’s case would need the services of different agencies however there are high chances that she would not be comfortable with being referred to some of the agencies. For instance she would need a doctor to attend to her for the visible injuries and also for some which have not been discovered. However, it can be predicted that the main barrier would be Teresa’s objection. Teresa would need all the agencies services so that she can get helped out of the crisis. She needs to be confident with the services as at worst, some of the services would need to be free in consideration of her situation. Intervention plan The intervention plan would begin with preparing of a visitation of a doctor. It is a priority to put the client’s health first and thus immediate arrangements would need to be made in order to see a doctor. Teresa would need to be in the doctor’s hand for as long possible in order to receive full treatment. All the doctor’s recommendations would have to be taken seriously (Nystul, 2015). To ensure that this is achieved, it would be important to have all the information from the doctor which would be kept confidential with Teresa and the doctor. The intervention effectiveness would be evaluated after the doctor’s recommended period where it would be determined whether Teresa has fully recovered. COORDINATION OF COUNSELING SERVICES 4 Secondly, immediately after she has seen the doctor, there is the need to meet Teresa and her boyfriend. It is necessary to get to the bottom of the conflict between the two and it would be aided by having both on the same table. Teresa’s boyfriend somehow would be the one to blame for Teresa’s injuries however over-inclination on her side is not the way to go. Honesty would therefore be demanded where Teresa would be required to share her entire story and how her boyfriend has been treating her. On the other hand the boyfriend too gets to give his story. At the end of the discussion the effectiveness would be based on the relevant facts that would have come up which would aid in the determination of an ideal plan of action necessary to end Teresa’s crisis. At the point of decision, the possible alternatives are either to provide the means for the two to solve their differences and continue living together or look for a different house where she would move to and live with her children. If the option would be to find a house for her, then it would be important to first look for means to get her a job but it would be ideal to solve their differences and have them move to a more spacious house. Service sector of business is the most critical segment in production. Customer
satisfaction is the key orientation in service delivery. In order to ensure that there is effective
coordination between the agencies in the community it has to be put in place. The aspect of
confidentiality in service delivery is one of the most crucial elements. In most cases it is common
that employees breach on the agreement of confidentiality even without being exposed to
coercive situations (Godwin, 2015). This breach of agreement is not only subject to penalties but
it jeopardizes the reputation of a firm and may lead to lose of potential clients because they don’t
have confidence on the firm to keep its confidential information.
As an employee, it is your responsibility to ensure that client’s confidential information is
not disclosed at any cost. Failure to which it translates that the employee’s social work ethics are
in question and can be fired in some cases. Maintaining confidentiality with clients proves our
integrity whereby as an employee one must be trustworthy not only to the clients but also to the
family and the community that we seek to serve. Secondly, confidentiality builds on human
relationship thus making the client more confident to give more useful and guiding information
on the forward or on the root causes of the problems.
