The U.S health care delivery system has evolved quite unlike the other systems in the
developed worlds. The entire system is a product of social, political, and economic factors,
which have resulted in a unique health care delivery system. Ever since the colonial time up to
the start of the 20th century, there was a big gap in medical advances ranging from experimental
research to medical training between America and other developed countries such as Germany,
France, and Britain.
During the Pre-Industrial Era (1700s to 1800s), the medical profession was still an
insignificant trade due to a number of reasons. First, the medical profession was in a state of
confusion. This was mainly because it was being treated as a trade as opposed to a profession.
For one to practice, they did not need any form of the rigorous curriculum of study, clinical
experience, residency, board exams, or even licensing. Consequently, anyone who deemed
himself or herself as a physician would practice with the needs of the marketing determining
whether one would be effective or not. Even commodity merchants engaged in medicine by
selling herbal remedies, tonics, and cathartics. Midwifery and homeopathy was practiced freely
with barbers acting as surgeons. Therefore, there was a great confusion as to whether the
medicine was a trade or profession as there was even no basic minimum training level for one to
practice. Secondly, the period witnessed the use of primeval and basic medical procedures such
as bleeding, purgatives, use of emetics and purging with enemas were common methods of
clinical treatment. Thirdly, there existed no essential medical institutions such as clinics, nursing
homes, and even quarantines for those with contagious diseases. Fourth, the demand for

professional medical services was unstable especially in the rural areas. This mainly because
medical care was being offered by individuals who were not medical doctors. Finally, yet
importantly, the medical education during the preindustrial period was also substandard. In fact,
