1. What was accomplished this year (e.g., results, competencies (including compliance, skills, and
feedback from others?
Patient relationships
Areas of accomplishment for this year include building and establishing a positive rapport with patients.
This was imperative as I started the new position during the year and with various barriers.
Quality of Life promotion/ patient
Supporting patients in re-engaging in the transplant process during the pandemic. There is evidence by
reviewing Facility Health Records for the past few months.
Adaptability
Performing Covid-19 screenings on a regular basis. This ensures patients entering the building safe and
overall safety of patients. Performing this duty requires time and constant re-adjustment of time and
duties for screenings to be complete.
Care Delivery
Despite the prevalence of the pandemic, kidney treatment and dialysis have been going on as
scheduled, and care delivery has improved.
Feedback
Referrals are a mode of indirect feedback, and though the result is wanting, it is evident of the hospital’s
performance.
2. What could have gone better? (include top three areas of opportunity and note areas of
underperformance
In light of the challenges:
Workload over 140 patients
Volume: Patient needs are high, especially as general services are limited due to closure and shorten
hours.
Improved performance; Calverton dialysis could have done better; this is in light of the performance of
Mt Rainer and the available gaps in the several assessment metrics. The patient census was low, at 39,
which is below average. Need to improve on the individual village and division performance
Patient education: This metric is also staggering and needs immense improvement to ensure the
patients are better equipped to accept and receive treatment.
Referrals: were also less, which is a deficit between the care provided for each dialysis session and the
overall service offered to the patients.

At DaVita, the culture, core values, mission, and vision are embedded with the ability to provide care;
hence their adherence is also considered an evaluation metric.
In light of the DaVita way, hospital culture is seen through these processes; just like a good village,
everybody is a team working for the common good. Through the performance analysis and regarding
the set metrics, the hospital has given life to many. The patients are also part of the hospital.
