Introduction
Hello, my name is (Student Name) and in this presentation, I will be taking you through
care coordination, its importance, the role of nurses in care coordination as well as change
management systems. The presentation will also look into ethical rationales within care
coordination.
Care Coordination and Effective Collaborative Strategies
Care coordination is the synchronized efforts of patient care activities and information
among various healthcare providers to increase efficiency in service delivery for the benefit of
the patient. This sees to it that all healthcare providers know their role as well as the patients’
needs and preferences and how they all come together in the bigger picture to ensure better
quality of life for the patient.
Care coordination relies on collaborative efforts among the inter-professional team that is
patient-centered. Seeing that the patient’s well-being is the common goal uniting the different
healthcare workers, for effective communication, the basic and yet the most crucial part of the
collaboration process would have to be the patient, (Sugavanam, T., et al 2018). Patients need to
be actively involved in their wellbeing and this can be done by educating them on things that
they can do on their end, simple goals such as cutting out on junk food for a diabetic patient. It
also empowers the patients to be their own self-managers and increases engagement as they are
able to easily point out and communicate should any complication arise.
Having an accountability partner for the patient in the form of primary caregivers such as
family leads to a more comprehensive effort in the patient’s health. This also reduces any delay
in communication in the event that a patient is incapacitated and needs urgent help. If a member

of the family has been acting as a caregiver or accountability partner for the patient, they are able
to give a better detailed brief on the patient’s history and even shed some insights on their
treatment plan. The familiarity of having a member of family to help in care other than a
professional also psychologically works to ease a patient into their treatment plan and therefore
results into better health outcomes, (Sugavanam, T., .et al 2018).
Advances in technology allow for real-time communication which comes in handy in
care coordination. Among the various inter-professionals, there is need for open communication
and reduced response time. By using electronic medical records, a patient’s updated health report
can be posted for other healthcare practitioners helping in the treatment plan to access
immediately. They can then act on the information given and identify how the new updates alter
or affect their part of the treatment. Yousefi notes, that the almost instantaneous mode of
communication results in prompt action and as data is interpreted real time not increasing
efficiency but also health outcomes as in one sitting, the patient is able to access multiple
providers without too much back and forth (2019).
Aspects of change management that directly affect elements of the patient experience essential to
the provision of high-quality, patient-centered care.
