NAME(S): Lucy Johnson
DATE OF BIRTH: N/A
PRIMARY LANGUAGE: N/A
REFERRED BY: Self-referral
INTAKE DATE: N/A
EVALUATED BY: Teresa Rudnick
DESCRIPTION OF CLIENT(S):
Lucy is a young a 19-year-old female client. She is also Caucasian. From our first encounter, she looks to have grown
very attractive but looks tired and appears to have tears in her eyes. However, her behavior seems peculiar since she
seems happy and sad at the same time. However, my overall impression is that she has greatly improved since her last
visit, but she looks stressed.
PRESENTING PROBLEM:
Based on the session that I had with Lucy, she seems to be stressed. Therefore, Lucy appears to be stressed since she
acknowledges that she feels emotionally pressured because she juggles two jobs. At the same time, Lucy also confirms
that she sometimes feels detached from her own body and sees herself from the outside. In addition to this, Lucy also
feels dulled almost every time and has felt overwhelmed for some time. At the same time, she has also been having a
hard time concentrating in school and has not been turning in assignments on time. As far as her functional impairment is
concerned, she is constantly worried and also experiences unreal feelings. From Lucy's perspective, she feels like she is
detached from the outside world, like she is entirely alone, although she doesn't feel like she has been depressed lately.
HISTORY OF PROBLEM:
While we have had a previous encounter with Lucy, her unreal feelings seem to have been on and off. While she was
about 13 years, her mother was upset all the time, resulting from her father's leaving, which made Lucy feel extremely
angered. As a result of her anger, Lucy spent more hours in bed while sleeping than she was ever awake for about a
week. Last summer Lucy also felt that her family was not supportive and her mother had emotionally hurt her after asking
if she thought she was too good for them. Currently, she feels like there is too much t...
