1. Give an example from the book where insufficient testing was a factor in a program
error or system failure.
Therac-25 was a machine that provided radiation treatment to patients who were suffering
from cancer in 1982. At that time, it was the most advanced cancer treatment machine since it
had an in-built computer to aid its operations. However, due to insufficient testing, the machine
ended up causing injuries and death to patients. Therac-25 released excess radiation, which
resulted in injuries and deaths (Leveson, 2017). Investigations into the incidents later revealed
that proper procedures for testing before release were not followed. Other factors included
viruses in the program used to control the machine, insufficient systems of reporting accidents,
and lack of follow-up into the reported incidences.
