Example: Community Health Nursing Diagnosis: Social Isolation of the Elderly: Increased risk of social isolation among the elderly population of xyz county (your county or city) related to lack of social activities due to the current pandemic as demonstrated by above average depression rates among the elderly. This statement clearly identifies the population of interest and the problem, but also gives clues to potential experience activities: Would more interactive activities available on social media be a solution? Or are there safe methods for the elderly to attend activities in the community? Is there adequate bus or transportation for the elderly? Refer to: o Section 2.1 in your course material for assessment of the primary prevention topic. (B1) Health Inequity/Disparity Revised 8/3/2020 3 For the section on Health Inequities, you will identify imbalances of service within your community and provide data to support these imbalances or inequities. Think about your general population and populations at risk related to your health concern. For example, from your Field Experience did you identify any vulnerable populations of the community who were at a disadvantage for services? Was insurance, access to services, income, location, lack of understanding contributes to some individuals not receiving higher levels of resources? You will back up your findings with data from your community. The data can include statistics supporting poverty levels, transportation services, insurance or lack of insurance, organizations available to community members, etc.…… Refer to: (for information on health disparity) o https://www.cdc.gov/nchs/ppt/nchs2010/41_klein.pdf o National Health Initiatives, Strategies, and Action Plans (Link found in 2.1 in Course Material) o Clark textbook, page 148 - Health disparity definition Explore HP2020 • Defining and measuring inequities/disparities • Social Determinants of Health (B1a) Primary community and Prevention Resources In this section you will discuss the significant resources you found in your community during the field experience to support prevention/promotion of the health concern. Williamson County Community Health and Population-Focused Nursing Field Experience
(B) Community Health Nursing Diagnosis Statement
Health is a fundamental concept in societal development and forms the basis for change
and improved level performance. Ensuring that health needs within a given setting are
maintained builds a broader foundation for change through economic progress. However,
health challenges are detrimental to societal growth; hence, it is essential to efficiently identify
and control them. The problem that I will consider is the increased infections in Williamson
County (Wang et al., 2020). Coronavirus disease has been a significant health concern in
Williamson County, forming a significant health challenge. The commitment to improving
change development presents a broader basis for understanding approaches to improve care
within the social context. The fast spread of the covid-19 disease has demonstrated a strong
base for implementing interventions that can help protect the health status of the community.
The risk of covid-19 in Williamson county among adults has increased significantly despite
the control measures that have been implemented. The increase in infections has been linked
to noncompliance with the predefined covid-19 control measures among residents (Harris,
2020). These measures include masks, social distancing, avoiding crowded areas, and washing
hands regularly. Therefore, it is integral to implement better measures that can help control the
rate of infections while improving the local community's health needs.
(B1) Health Inequity/Disparity
Williamson County in Texas is a predominantly white community accounting for 72%
of the total population, followed by Asians with 8% and African Americans at 7%. Currently,
the county has approximately had a population of 613 104. The number of people between
males and females is comparable, although the women population is slightly higher, 50.8% and
49.2%. More than half of the population is aged forty years and above, where 21% are aged
above 50. The rate of covid-19 spread is highest in older adults, which illustrates that most

residents in Williamson county are at increased risk of adverse effects of covid-19 if there are
no significant interventions implemented to improve adherence to Covid-19 prevention
protocol. There are approximately 222,664 households in Williamson County with 166 396
families. Each home has 2.69 persons, while 42% of the families have children. The immune
system of children is weak, which makes them highly vulnerable. Around 4.15% of the families
in Williamson county live below the poverty level (County, H. W. (n.d.).
The rate of Covid spread in the recent past has been increasing, with daily reports
showing 293 cases reported every day. Currently, there are around 42,838 confirmed covid-19
cases within Williamson county, with 493 reported and confirmed deaths. The uptake of the
covid-9 vaccine has also been moderate, with a current report showing that 61% of the
Williamson county population has been fully vaccinated.

There are 534 hospital beds

available, with only 16 intensive care unit beds available, representing 2% of ICU beds in the
county. By 30th July, there were approximately 29,614 active covid-19 positive cases, causing
significant strain to the available resources in the county (Hinds, 2021).
This shows that the county is on the verge of a disaster since a surge in cases is likely
to lead to overwhelmed healthcare. Therefore, it is imperative to focus on building a strong
basis that can be effectively utilized to improve healthcare quality and ensure that the local
community embraces Covid 19 prevention guidelines. The number of individuals who have
been vaccinated is moderate despite the county currently not managing any waitlist, and any
individual wishing to receive the covid-19 vaccine will schedule an appointment directly with
the provider of their choice (Traynor, 2021).

B1a: Primary community and prevention resources

Williamson county has immense resources which provide leverage in dealing with
Covid-19 implications. However, it is essential to understand that controlling the spread of
community is a multifaceted activity that includes the efforts of everyone. Within the county,
many hospitals have been at the forefront in ensuring that all covid-19 cases are taken care of
efficiently without a drop-down in healthcare quality. The leading hospitals within the county
include Rock springs, Round Rock Medical Center, Ascension Seton Williamson, Brushy
Creek Family Hospital, Baylor Scott & White Medical Center, and many others that are helping
in hospitalization and vaccination of the people. Additionally, numerous websites have been
created to assist in placing a booking for appointments (Courtemanche et al., 2021).
Recently the county has put in place measures to help in enhancing vaccination through
the family health system and curative systems, which have been essential in increasing the
uptake of Covid-19 vaccination. Due to the trauma experienced from the pandemic, there are
many mental health services around the county. The county also has programs in helping the
needy community with housing and food. Some families, due to Covid-19, cannot be able to
provide for their houses nor cater for the bills. Most of them were rendered jobless due to the
pandemic. Some were fired to reduce the number of workers, while some lost their businesses
(Hinds, 2021).
Through the local authorities, the county has also focused on maintaining a higher-level
commitment to all covid-19 protocols in line with the State of Texas. Enforcing measures such
as wearing masks and maintaining social distance have been implemented to a varied level of
success within the county. The implementation of stay-at-home orders was executed within the
county. All those affected were taken care of through the economic stimulus program, which
has been active within the county and created an improved basis for change and implementation
of favorable outcomes (Wang et al., 2020). The vulnerable groups, including the homeless,

poor, and household with children and elderly, have been prioritized considering the increased
risk of infection in this population.
(B1b) Underlying Causes
The current health concern that the Williamson county community is grappling with
currently has been an increase in Covid-19 infections. There has been reduced focus on
adhering to the covid-19 protocols. During the first wave of Covid-19, the rate of deaths across
the world was worrisome, and many people developed fear since they did not know what to
expect or when they were likely to get the disease. The hospitals were overcrowded, and more
than 2,000 people were dying daily in the United States. Many people were highly tensed and
were making efforts to ensure that they have minimal exposure and reduce the risk of infection.
However, since the vaccination was made available, there has been increasing laxity among
many people who violate the covid-19 guidelines.
In Williamson County, around 70% have received only the first dose of vaccination
compared to 61% who have been vaccinated fully. As a result, many are not well informed
about the functionality of the vaccination. Vaccination reduces the risk of adverse effects,
although it does not mean that those vaccinated cannot be infected with the virus or spread to
others. There have been challenges, especially with mixed reports regarding lessening the strict
covid-19 guidelines and open the economy. However, this can only be done effectively when
there is high uptake of vaccines across the population.
Sporting activities, restaurants, and other public spaces have been opened up to ease
the economy. Even though these places and activities have been opened focusing on the covid19 guidelines, adherence to these measures has been a challenge. The majority of individuals
feel that the impact of covid-19 is currently lower than the last 12 months. Lack of awareness
and health literacy has presented a significant challenge leading to the current situation.

Therefore, it is imperative to build change and help create an improved basis for change where
it is possible to build change and improve individual wellbeing (Courtemanche et al., 2021).
Health literacy plays a fundamental role in explaining specific processes that define
change and promote a higher level of change. A low level of health literacy is detrimental to
an individual ability to navigate through a highly diversified and complex health system, which
requires a well-organized context for a highly diversified change structure. A low level of
health literacy means that an individual does not understand the underlying challenges in
healthcare and how to navigate these challenges to achieve the intended positive outcomes.
Social and community contexts, especially in the Covid-19 period, have remained essential
components that define the level of spread and control (Hinds, 2021).
(B2) Evidence-Based Practice (Lesson 4 in your Cos)
The spread of Covis-19 has been attributed to many aspects. However, the role of
humans in the spread of the disease cannot be ignored. Compliance with the strict covid1-19
guidelines presented the basis on which it was easier to overcome the pandemic. However, the
tendency for people to flaunt these guidelines has given a problematic basis on which it would
be possible to improve health outcomes.
Li et al. (2021) sought to investigate the wearing of masks to reduce the spread of
respiratory viruses in a systematic review study. As a result, the changing behavioral pattern in
seeking care among people has shifted significantly, focusing on the utilization of masks and
social distancing measures considered the most efficient in controlling the spread. The study
further identified that masks have beneficial in reducing the spread of respirato...
