Aquifer Case Study: Family Medicine 13: 40-year-old male with a persistent
cough
Patient: Mr. Frank Dennison
DOMAIN: HISTORY
1a) Identify two (2) additional questions that were not asked in the case study and
should have been? And 1b) Explain your rationale for asking these two additional
questions.
During the assessment, some of the critical questions that the patient could have been
asked include: Do you cough after meals? Have you lost weight recently? Frequent
cough after eating could be a manifestation of an underlying condition rather than a
natural reflex. For instance, gastroesophageal reflux disease (GERD) is associated
with acid reflux into the esophagus and trachea, thereby triggering cough associated
with a sore throat and hoarseness (Lai et al.,2021).it is also essential to ask a patient
who presents with chronic cough whether they have witnessed any unexplained
weight loss. Weight loss can coexist with symptoms such as chronic cough in patients
with lung cancer, M tuberculosis infection, HIV, and COPD. Thus, a healthcare
provider should ascertain whether the patient has had an unexplained weight loss. The
current guidelines for managing cough highlight the need to take a careful history to
establish specific features relating to cough in terms of its duration, typical triggers or
aggravants that are associated symptoms (Kanemitsu et al.,2019).
1c) Describe what the two (2) additional questions might reveal about the patient's
health.
Another two questions involve assessor might ask the patient include any presence of
trouble swallowing and any history of postnasal drip. Difficulty swallowing
(dysphagia) may be associated with pain. The swallowing during the pharyngeal

phase in patients with chronic cough reveals can indicate the possibility of aspiration
pneumonia. Postnasal drip happens when secretions from the nose move into the
throat, thereby causing irritation or inflammation that culminates in cough. In most
people, these secretions observed in postnasal drip occur in people with sinusitis,
allergies, allergies, and rhinitis.
