Description

Response to Sierra Simmons

I developed the competency for nursing theory development by discussing different nursing theorists and the framework of their theory. I specifically learned about the Behavior System Model of Nursing, developed by Dorothy E. Johnson, which classifies a client as a behavioral system that is influenced by internal and external environmental factors which contribute to health promotion and preventing illness. By assessing whether or not an imbalance is present within the subsystems, nurses can determine how to restore and maintain a balance between the environments and subsystems. I can apply Johnson's theory in my future practice when providing multidimensional care to the patient (behavioral system) is composed of interrelated subsystems that are influenced by various environments (physical, mental, social, cultural etc.) By assessing the patient's environments, I can gain insight to help determine the proper interventions for their subsystems.

In module two we continued to learn about theories that have shaped nursing practice. I chose to research two different theories that were developed by Nola Pender and Myra Estrin Levine. Nola Pender, who developed the Health Promotion Model, which focused on modifying unhealthy behaviors to promote health and enhance the quality of life (Khodaveisi et. al, 2018). Like Johnson's theory, she believed that there is a direct correlation between a patient's environment and their behaviors. Levine's Conservation Model of Nursing centers around the nurse's ability to maintain wholeness and balance by focusing on the patient's responses to adaptations. Levine's model had a more proactive approach by promoting the use of preventative measures to prepare for the physical, social, and emotional adaptations patients’ may experience in response to changes. Both of these models can be applied in my nursing practice to help understand the key factors that influence a patient's behavior and health, especially if modified behavior can significantly improve their condition.

We dissected the different components of research by critically appraising quantitative and qualitative research articles. Quantitative research is objective and involves large study groups. Also, numerical data is collected and it often involves closed-ended questions and findings that can be generally applied to wide-based populations. Qualitative research is subjective and analyzes data gathered in the form of surveys, interviews that are based on an individuals' experiences. appraisal consisted of identifying the problem, purpose, methodology, and literature review of the research. We also discussed the framework of research as well as the research objectives, procedures, findings, strengths, and limitations.

I developed the competency to understand the relationship between research, theory, and evidence-based practice by using theoretical frameworks to help guide and develop my own research on an issue that will provide an evidence-based solution. Research provides the basis for the development of theories, and in turn, these theories provide the structure for new research that will yield the best evidence-based practice.

We learned how to propose an evidence-based solution by examining different issues within nursing practice and using research to formulate evidence-based questions. We used the PICO format to structure our research question and performed a literature review to gather evidence to support the need for change. My research proposal examined whether turnover among first-year nurses can be reduced by participating in a nurse residency program in comparison to first-year nurses who do not participate in a nurse residency.

Response to Kathryn Thomas In the beginning of this course, I learned about theory development through course readings and provided content. One specific piece of new knowledge I gained was regarding the different types of theories that exist. A few types of theories that are relevant to nursing are metatheories, grand theories, and mid-range theories. Metatheories apply to nursing through philosophical and methodological theories regarding nursing care. Grand theories apply specifically to nursing practice through structure and purpose. Finally, mid-range theories apply to smaller, more specific parts of nursing and provide ideas and variables that can be tested through research. This will apply to my nursing practice through being able to identify theories and the type of theory. Additionally, determining that only mid-range theories are testable will help me in creating and carrying out research by developing mid-range theories. Next, we learned about many different nursing theorists. I learned about Patricia Benner’s theory of moving from novice level nursing to developing expertise. I gained this new knowledge through the course readings and the nursing theorist proposal project. Benner’s theory about nursing experience outlines the process of nurses progressing through gaining knowledge and expertise in their field and to become experts in the care they are providing. This new knowledge will help me as a novice nurse to seek knowledge and experience to advance through the profession. Through the course material and lectures I learned eight steps to the research process. Those steps are to first identify a problem and formulate the purpose of the research. Next, researchers must identify methodology they plan to use that is appropriate to the research problem. Researchers should review the literature and identify previous research conducted on the topic. They can then develop the framework they plan to use as well as stating the objectives and procedures of their research. After completing the research, they will discuss the results and meaning of these results with their audience. Understanding the research process will help me in my nursing career if I am involved in conducting research and will help me to understand the process when reading research as well. In completing research, there are two main types of methodologies that can be utilized that we learned about through course content. The first is qualitative research. This type of methodology is subjective and can contain methods such as interviews. The other type of methodology is quantitative research. This type of research is objective and can provide insight to a larger population. This knowledge will impact my nursing career by helping me understand the generalizability of research findings. We discussed research, theory, and evidence-based practice in this class through the course content and lectures. I learned that theories help to guide research, and research is a part of providing evidence-based practice. Theory is developed and helps to create a hypothesis that can then be tested through research. Research along with patient beliefs and clinician expertise are the components of providing evidence-based practice. As a nurse, I can utilize theory and research to provide the best care to my patients. Another focus of this course was conducting research based on a problem and proposing a solution. I learned and practiced this skill through course assignments and lectures. One example of the knowledge I gained was regarding workplace violence in the emergency department setting. Utilization and implementation of learning modules regarding workplace violence can help decrease instances of violence against nurses. This new information and practicing the skill of proposing a solution to a problem will benefit me in my nursing career by allowing me to make changes within a department I work in. In this course I practiced researching topics and learned information literacy. In the world today there is information coming from many sources. It is important to have the skills and knowledge to appropriately search for pertinent information to the problems at hand. Specifically, I learned that having a good search strategy can produce relevant results. This will help me when I am searching for information in my practice.
