One of the crucial responsibilities of nurses has been to implement care coordination
activities. Care coordination plays a crucial part in the healthcare industry in ensuring patient
safety and positive patient outcomes. Additionally, the future on nursing report indicated the
need for nurses to integrate and work with different healthcare professionals to address health
equity and disparities issues. Care coordination plays a role in integrating nursing care with other
healthcare professionals. Therefore, care coordination plays a part in ensuring care for chronic
disease patients, but it will also help reduce healthcare disparities. According to Hannigan et al.
(2018), nurses do not focus only on physiological care for patients, but they ensure holistic care
for a patient such that their quality of life improves. Therefore, in ensuring holistic care for
patients, care coordination provides integrated care approaches that improve the health of the
patient and focus on reducing health care disparities and improving the quality of life of a
patient. Therefore, care coordination plays a role in enhancing healthy people 2030 goals focused
on reducing healthcare disparities and enhancing quality care for population groups
Healthcare Issues
Provision of care in the nursing field incorporates addressing community healthcare
problems in an integrative approach that allows for the care coordination process. Addressing
population healthcare issues using the care coordination process reduces healthcare costs and
enhances positive patient outcomes. Moreover, for nurses to address and enhance holistic
