The program focuses on three main risk factors: nutrition, physical activity, and tobacco use. The community you are working with is low-income, low-education, multicultural, and the aforementioned risk factors are prevalent throughout.

This discussion has two parts:

States. In the recent past, this condition has been the cause of at least 1 in 4 deaths in the
country as shown by recent studies (van Lieshout, Huntink, Koetsenruijter & Wensing, 2016).
