Objectives
▪ At the end of this lecture student should be able
to explain the etiopathogenesis,
pathophysiology, clinical manifestations,
diagnosis, treatment and complications of the
following conditions:
o TB Meningitis
o Encephalitis (HSV encephalitis, Acute disseminated
Encephalomyelitis- ADEM )
o Fungal meningitis
o Brain and spinal cord abscesses
TBM BACKGROUND
Most serious complication of TB infection, Usually
due to M. tuberculosis
Fatal without effective treatment, significant
morbidity even with treatment
In children CNS involvement occurs during primary
infection (rather than reactivation). TB of the CNS
takes two forms: TBM and tuberculomas
Usually results from hematogenous spread from a
primary focus (lungs)
Variable presentation, but usually onset is insidious
– (More rapid in infants and young children)
TBM BACKGROUND
Hematogenous seeding into the CNS, caseous
lesions form in the cerebral cortex or
meninges (Rich focus)
These increase in size, and sometimes rupture
