Referral and Inquiry on child’s parent calls, sociology homework help

Question Description

Discussion Topic

Children and youth can be referred for case management services for a variety of reasons. Some examples include problems with school, behavioral problems, developmental delays, adjustments to events such as divorce or the death of a parent, trauma associated with abuse and/or neglect and homelessness. When a child’s parent calls to obtain services for their child, they may have been referred by their child’s doctor’s office, school, or local social service agency. After reviewing Chapter 14 of your text and the scenarios below; practice documenting an initial inquiry.

First, select two clients from the examples below.

  1. Six-year-old child was referred by his teacher for severe attention and aggression difficulties. Normal interventions for behavior change have been ineffective. Child’s father is calling for services.
  2. Two-year-old child was referred by pediatrician for social skill delay resulting from previous neglect by birth parents. Adoptive parents are calling for services.
  3. Ten-year-old child was referred by pediatrician for displaying some regressive behaviors. He has been wetting the bed almost nightly and having nightmares. Child’s mother is calling for services.
  4. Fifteen-year-old youth is several months pregnant. She has managed in school but only reads at the sixth grade level. She is planning on dropping out after she has the baby. Youth’s mother is calling for services.

Second, develop your client and your client’s problems. You can review articles online to find issues that your client might be struggling with. You can also think about how the client’s parent might be feeling and what issues the client’s parents might be facing when they make the first call. You will be creating information about your client in order to complete sections of a new referral or inquiry form. Third, complete the following sections, for both of your clients, from a New Referral or Inquiry form (It might be helpful to review Figure 14.1 on page 281 of your text): ATTACHED

Do two of these for the scenarios above.....

Referred by:

Chief Complaint and/or description of problem:

Previous evaluation, services, or treatment:

Taken by:

Disposition:

Finally, discuss with your classmates how it felt to complete your first referral. Were there any sections that you struggled with?

EXAMPLE Fifteen-year-old youth is several months pregnant. She has managed in school but only reads at the sixth grade level. She is planning on dropping out after she has the baby. Youth’s mother is calling for services. Referred by: Self-referral Chief Complaint and/or description of problem: Julie, the youth’s mother, called today, because her daughter was suspended for the 2nd time this month today. Her daughter is pregnant and is having a hard time keeping up with her studies. Julie (Youth’s mother) sounded very anxious to get help for her daughter, before her granddaughter is born. Previous evaluation, services, or treatment: Was seen by Dr. Mary Stevens, Roseville Health Clinic, from 3/7/17 to 8/7/17 for general anxiety. Taken by: Chanel Striffler Disposition: Referred to Chanel Striffler for intake 6/14/17 Two-year-old child was referred by pediatrician for social skill delay resulting from previous neglect by birth parents. Adoptive parents are calling for services Referred by: Mary Rogers M.D. Chief Complaint and/or description of problem: Larry, the adoptive father of 2year-old Zack, called in today, after having to pick up his son for disruptive behavior in daycare for the 3rd time this week. Zack has experienced neglect in the past from his biological parents, which has caused him to experience a delay in social skills. Father seems confused, but cooperative. Previous evaluation, services, or treatment: Received counseling services through Pediatric Medical Associates, from 3/13/17 to 9/5/17. Taken By: Chanel Striffler Disposition: Referred to Chanel Striffler for intake 6/14/17 So everyone, I completed my first referral. It was very scary at first. I was nervous that I would fill out something incorrectly. If I was this nervous to fill out a referral form for a fictitious client, I can only imagine what it would feel like to fill out a form for a real client. I’m sure I will be even more nervous when that time comes. As I completed the form, I became more relaxed and felt more confidence in myself as I made my way through the different questions. The only part of the discussion that I really struggled with, was the Chief Complaint section. I struggled with this part because I know our text told us to keep this part brief, so I found it a bit challenging to keep it brief while at the same time not leaving anything out. I enjoyed this discussion very much, because it gave us some insight on what to expect when we actually get into our career field. ...
