For the years that the company has been in existence, one can say the company has enjoyed its fair share of successes and failure. The company operates at 103 LTAC hospitals and about 20 rehabilitation hospitals located in 27 states with 1,611 outpatient rehabilitation clinics in 37 states and the District of Columbia. The company tend to attend to all sought of rehabilitation and long term acute care issues for the company to continue providing services according to its mission statement, they will be a need of re visiting their organizational leadership style and its leaders. Purpose of Proposal The purpose of the proposal is to address a critical issue that is affecting the quality of care that is been provided by the staff at Select Specialty Hospital. The issue here is basically the high turnover rate at the hospital and how reluctant staff nurses have become when providing quality care. The main concerns here how management have refused or doing little to address some concerns of their employees. This has caused many good employees to leave the company to other facilities regardless of the amount of money being given to the employees as bonus. The company also seeks the help of agency personals to provide services to the patients of the hospital whenever there is a shortage of staff. This trend has caused the company to lose a lot of clients since these agency staff personals are not providing quality care to the patients since they are not permeant employees of the company. Due to this, it is difficult to hold the agency staff personals accountable for their actions. CAPSTONE PROJECT 4 This proposal is to take a critical consider the organizational style of the company and the leaders as well. the proposal will be able to provide answers on whether the organizational leadership style is to be changed or some leaders of the organization should be re train or let go from the company. Method of Gathering Data Contracted statistics agency will concentrate on gathering information form patients and staff members of the company. This information will be gathered from surveys and questioners that will be provided to the patients and the employees of the hospital. This will help the company to get some answers and implement solutions that will be derived from the data gathered. With this, the company will be back on course in satisfying the company’s mission statement which is to “provide the highest quality health care and cost-effective outcomes by providing specialty inpatient acute care services to those we serve while providing a positive and fulfilling work environment for the staff”. Problem Statement There is a tremendous need for the organizational heads to address certain key issues that are currently affecting the company especially the staff high turnover rate. It is obvious that the amount of salary paid to employees is not the reason for the high turnover rate as staff turn to leave the company to work for its competitors for less money. For the past two years that I have worked in this company, there have been numerous times that I have wanted to leave not because of money but due to the nature as well as culture of leadership in the company. According to Schneider, (2002) organizations tend to develop the leadership that best suits them at a point in time but when that type of leadership turn to hurt its employees, the company tends to lose out on skilled staff members. CAPSTONE PROJECT 5 Due to this leadership style exhibited by the immediate leaders of the company, the company is experiencing a shortage of staff and as a result are patients receiving low quality care. This is because leadership appears to have turned deaf ears to the cry of staff members and has instead resorted to employing agency staff to make up this shortage. What the company is refusing to realize is that, it cost twice as much to hire the services of agency nurses to perform nursing and other duties for the organization. Agency staffing is very important since they cover certain areas when needed but one may forget that these agency staff are mostly not familiar with the patients already on the floor and their needs, resulting in the low-quality care the hospital is providing to the patients now. Over the past two years, patients who are mostly referred to our facilities by their case worker have often refused to be admitted by our facility, these patients have opted for different facilities that are located hours away from our location. This situation has also resulted in low admission rates at the facility which has caused most staff members to take low census days from work. Most staff members have lost their jobs due to those current situations at hospital. This issue has had a significant impact to the company’s revenue since 2015 and I am proposing an extensive review in the organizational leadership and culture of Select Medical. Organization background Select Medical hospitals dwells much on these core values which over the years has helped shape the existence of the company, these core values are; Superior quality and care, Compassion and Respect, Results oriented, Teamwork and Resourcefulness. Select Medical aims to provide the best quality health care and most cost-effective outcomes in specialty inpatient acute care services to patients, as well as a positive and fulfilling work environment for CAPSTONE PROJECT 6 the staff in line with its mission statement. The hospital´s vision for the communities it serves is to provide an exceptional patient care experience that promotes healing and recovery in a compassionate environment. This proposal aims to show an internal and external support for a change in the organizational culture and leadership of the company. This need for change in both the organizational culture and leadership of the company is because of drastic increase in the number of staff leaving the company after a brief period of employment (three months), even when huge bonuses are offered to dissuade them. As a result, there has been numerous complaints of low quality care being offered to patients. The high turnover rate according to staff is due to the relationship the organization has with its staff, for instance nurses employed by the organization and other members of staff have over the past years complained bitterly about how management puts the interests of the patients above the employees. It is true that the wellbeing of the patient is important since revenue is made through services provided to them but it is equally true that the interests of patients suffers when inferior quality care is provided by the nurses or other employees who do not feel motivated to give their best, this works against the company itself. Select medical was founded in 1996 and the company has focused on providing superior, patient-centered care that improves quality of life till this day. As of 31st December 2016, the company has locations in 46 states and the District of Columbia. The company has over one hundred and three LTAC hospitals and twenty medical rehabilitations hospitals in 27 states, it also has one thousand six hundred and eleven (1,611) outpatient’s rehabilitation clinics in 37 states and the District of Columbia. As of March 1st, 2017, the company can boast of having employed over 41,500 employees across the nation. The site I work for in Ohio has a total of one thousand two hundred (1,200) employees spread across the state. The company also enjoyed an CAPSTONE PROJECT 7 annual income of $3.64 billion dollars in 2016 which is a total decline from the revenue earn in the previous year (“Select Medical income-statement 2016.”). Below is a graph of the company’s annual income from 2008-2016; The hospital that this proposition is intended for is directed by an advisory group which is made up of the chief executive officer, chief nursing office, chief operating officer, compliance officer and lastly a quality control officer, it also has eight officers and nine executive members of the leading body of trustees. The hospital has an excellent opportunity for growth and providing excellent quality care for the community, according to the company’s website, the hospital has since 1996 involved themselves in countless volunteer opportunities in the sites or states they are located. Significance of the Study There are many health care organizations in the world and most of them are still striving to be able to be reliable or to attain high reliability. High reliability is the ability of a healthcare facility to be able to overcome the challenges that healthcare facilities face for example the CAPSTONE PROJECT 8 problem of expensive health care facilities in the United States. A healthcare with high reliability will overcome the challenges and at the same time be able to offer the best possible care to patients. Leadership has previously been referred to the top leaders but those that matter are the frontal workers and supervisors are the ones that are close to patient-centered needs. The importance of the topic to the writer This is a very important topic as it deals with the most fundamental problem that faces the United States that need to be addressed. America’s health care system is the most expensive health care system in the world and by writing about health care system in general, there will be additional information acquired from the study and research carried out that may be important for the America's healthcare (Hernandez & O'Connor, 2010). The significance of organizational leadership to healthcare Leadership can also be defined to mean the way an individual behaves when driving a certain group of people towards a common goal that is shared. Such a big organization is prone to conflicts between the different branches of the organization hence leadership should ensure to capitalize on the diversity of the different groups in the organization to avoid conflict. There are different forms of leadership that a leader may choose to apply in his leadership; Transformative leadership is a theory that insists that people work more and they are more driven if they have a mission that they are working towards. Transformative leadership CAPSTONE PROJECT 9 necessitates leaders to be able to communicate their mission, vision, and goals in an exciting manner that will motivate workers to do their job with more commitment and with a mission. Collaborative leadership is when people work together in collaboration with a common goal to be achieved. This kind of leadership requires that the leaders communicate to the workers and share knowledge so that there is reduced complexity in the healthcare organization. Global Healthcare Organization Leadership Significance Good leadership is the one that ensures that business is done in the organization and how it is done matters more than how much the work is done. Global organizations should ensure that they carry out leadership programs that are helpful to the organization's development. Works should be given the power to make their own decisions. Leaders should not be supervising each step of the work, workers should be allowed to make their own decisions without the interference from management. Employees should be inspired to work because inspired employees are the ones that will do everything possible to achieve the best for the organization. American healthcare system can emulate from the other healthcare systems globally for example Germanys to help it provide the best healthcare at a cheaper price (Page, et al., 2004). In conclusion, leadership in any organization is very important as it is what will either take the organization to great heights or break it. Identification of Issues Leadership is the process through which other people are influenced by an individual to accomplish organizational goals and that individual directs the business organization towards enhancing organizational cohesiveness and coherence. The individual who influences others or groups to achieve the set goals is referred to as a leader. Leadership is a process is learned but the CAPSTONE PROJECT 10 knowledge, as well as the skills possessed by a leader, is greatly influenced by the leader’s attributes or traits. These attributes may include ethics, values as well as the beliefs and characters of the leader. The factors of leadership are categorized as; situational factors, followers, leader and the communication of the information which is crucial in running the health care facility. Poor leadership is the toxic leadership which when left unchecked or controlled may compromise the values, promote non-compliant behavior as well as norms of the healthcare facilities. This poor leader influences the organization variables negatively and it thus becomes imperative for the stakeholders to understand how such a leader affects them indirectly or directly and identify the various techniques and methods they can adopt in mitigating and eliminating these effects (Schilling, & Schyns, 2014). Poor leadership tends to harm the company as well as the people in the organization. The leader, therefore, affects the organizational culture negatively through engaging it in behaviors that are self-destructive and compromises that leader’s reputation and the organizational values. This is the failure of adhering to the normative organizational standards (Schilling, & Schyns, 2014). The employees in the healthcare facility, on the other hand, internalize these norms and the values of the organization as well as those of their leader. Healthcare facility is a value-based institution that requires the employees to adhere to the culture, norms and the ethical guidelines always. In most cases, the impact of the toxic leader on the subordinate’s performance may be greater especially for those subordinates manages to identify the keen sense and meaning in their tasks that they handle. The negative impact can be because of the acts committed by these poor leaders directly or those acts omitted by the organizations. These may include unethical CAPSTONE PROJECT 11 behaviors and failure by the organization to put corrective measures in place to deter the toxic behaviors. In a healthcare facility, the nurses need to follow the directives from the doctors. In relation to this, a poor interpersonal relation in a workplace can be clearly identified as the major source of stress. Leaders who are abusive stress individuals who in return may be isolated by their workgroup at the workplace. This may impact to less job satisfaction on the employee’s view. Due to job dissatisfaction, employees leave the organization. Poor leadership results in the erosion of ethics in the organization. This is because it degrades the trust and values that are inherent in the culture of the organization, the acts of employee’s harassment which may increase. The harassment behaviors which include ostracism and threats undermine the organizational values and tarnish the profession and the reputation of the organization. It also erodes the trust that binds all the employees and then organization management. Harassment in the workplace has serious negative effects especially on the employee’s morale as well as creating a culture that is toxic for devaluing civility and the mutual respect. Moreover, the leader’s negative behavior may trigger negative responses which further trigger the reactions that negatively damage the organizational teams or individuals who tend to react in a way that reinforces the bad behaviors. The organizational corruption, therefore, manifests as the willful distortion of the orders. This results in moral deterioration (Schilling, & Schyns, 2014). CAPSTONE PROJECT 12 Discussion of the Issues i. Poor leadership causes stresses. What are the sources of stress in the healthcare facility, effects, and the coping strategy? Stress refers to a term that describes any biological responses to the physical mechanisms. Stresses especially in a healthcare facility may be caused by time pressures and performing the tasks with tight time deadlines. They can also be caused by poor working conditions such as noise, poor lighting, and the internal factors which relate to a given profession, the roles of an individual within an organization, the culture and organizational atmosphere as well as poor leadership which negatively affects the employees’ performance. These stressors can be reduced by ensuring that good working conditions are put in place. The reason for caring about stress experienced by employees is because, in a healthcare facility, they are exposed to many stressors due to the numerous people they handle. If these stresses are not controlled, it may result in physiological, emotional and behavior disorders. Employers should reduce the stress related to work, enable the employee to gain social support and implementing stress coping programs (Mellott, 1997). ii. Why are ethical principles imperative in the healthcare facility? Ethics in a profession is the hallmark. They help the employees especially in a healthcare facility to treat the patients individually, ensure that they promote benefits to them, and ensure that they respect the dignity of their clients-patients and their own dignity. Ethics in an organization creates a culture which transforms the resources of an organization into its success.
