Introduction
The Vila health media scenario represents health care providers from various settings who
collaborate in planning for the medical care of Caitlynn, a two-year-old patient hospitalized since
she was diagnosed with pneumonia. Since it was her second admission within the last six months,
the health care team planned to perform more tests on her to examine the immediate cause of the
current condition. The completed tests revealed that Caitlyn is suffering from cystic fibrosis. This
was confirmed after performing a sodium chloride test. Dr. Copeland, who is the team leader in
the health care facility, offers critical guidelines to health care providers within and in other
hospitals to ensure better health intervention measures are implemented to promote better health
outcomes to the patient.
Effective collaboration between different health care providers indicates an excellent
example of remote collaboration. The health care team uses Skype to share information with other
stakeholders in other facilities. This discussion will focus on improving the safety and patient
outcomes based Vila health scenario through creating proposed evidence-based model care plan
while identifying positive effects on patient results. The discussion will also involve reflecting on
accurate evidence of the proposed care plan and determining the advantages and approaches to
address challenges associated interdisciplinary collaboration among healthcare teams.
Evidence-Based Care Plan to Improve Patient Safety
Before creating the plan for the care for the patient, Dr. Copeland and the team ensured
that the health care providers were managing the suitable health condition. The physicians, nurses,

on reducing the severity of the symptoms and reducing related complications. Therefore, the
evidence-based care plan that I find to be effective in improving patient safety is the ability of the
health care team to evolve and individualize patient care; this will be effective in ensuring adequate
