1 Mental Health in Adolescence Dagny Amigo Capella University NURS-FPX4900 Kyle Savitz 02/18/2022 2 Mental Health in Adolescence Population health outcomes depend on various risk factors and evidence-based solutions implemented with the population. Nurses and nurse leaders should do more than merely providing care to people presenting with different health conditions. They should proactively identify patient and population health issues, research current evidence on addressing them, and implement evidence-based practice to address those concerns. The focus of this capstone project is adolescents as the primary target group. Adolescence is a critical stage in life and development whereby significant life changes occur and the adolescent may experience additional risks to physiological and mental health outcomes not experienced during childhood. This paper outlines the current literature on mental health for adolescents and necessary leadership and collaborative strategies to address the health issue. Population Health Problem The population health problem to be explored is adolescent mental health, especially depression during adolescence. Adolescents face many challenges including peer pressure and biological changes that may affect how they experience the world, consequently affecting their mental health. Depression, anxiety, and other behavioral disorders are some of the leading causes of death and disability among adolescents and the rates of suicide have been associated with relatively high depression rates in the age group (Gijzen et al., 2021). It is important to address mental health in this population group to promote their well-being and resilience to reduce the risk of mental illness and other connected complications of the same. This capstone project will seek to address mental health challenges among high school students to promote health outcomes and reduce mental illness. 3 Mental illness is a significant challenge for this population and hence it deserves attention from a nursing perspective. According to the World Health Organization (WHO), one in seven people aged between 10 and 19 has a mental health disorder (WHO, 2021). Depression has become a major challenge especially among girls. 13% of all teenagers have had a major depressive episode in the last one year and girls are more likely (20%) than boys (7%) to experience depression (Geiger & Davis, 2019). These high rates of mental illness and especially depression are also connected to drug abuse and suicidal tendencies. As a result, mental illness is a multidimensional health issue for adolescents. The high prevalence of mental illness among adolescents requires attention from nursing staff and other healthcare professionals to minimize the impact of mental illness on overall health outcomes. Moreover, addressing mental health in adolescents is relevant to the baccalaureateprepared nurse because it aligns with the nursing essentials of practice. The nurse has a responsibility to implement preventive care to improve population health outcomes. Nursing essential VII states that “health promotion and disease prevention at the individual and population level are necessary to improve population health and are important components of baccalaureate generalist nursing practice” (American Association of Colleges of Nursing, 2008). Focus on the mental health and outcomes in adolescents is a form of health promotion and disease prevention. The aim of the project is to reduce the incidence of mental illness in adolescents and this reduction can be achieved through proactive prevention and promotion of mental wellness in the target population. 4 Evidence Supporting Nursing Actions Research on adolescence mental health has provided many evidence-based solutions that could be adopted in primary healthcare. First, research supports the existence of a serious mental health issue among adolescents. Loneliness and social isolation have been linked with higher levels of depression and mental illness in adolescents and young adults (Christiansen et al., 2021). However, as observed in practice, it is not often that teenagers are assessed for feelings of loneliness and social isolation. This is a gap in practice that should be addressed. A systematic review focusing on mental health during COVID-19 found that adolescents have increased levels of anxiety and depression as well as cannabis and alcohol use during COVID-19 pandemic (Jones et al., 2021). These are just a few conditions that may be affecting the mental health epidemic among adolescents but the literature is clear that adolescents are an at-risk population when it comes to mental health outcomes. Moreover, evidence has shown that significant barriers exist in implementing evidencebased practice (EBP) to address mental health issues among adolescents. One of the most significant barriers is organizational and provider capacity (Damian et al., 2018). Failure to ensure availability of mental health services and failing to train staff on mental health interventions could lead to suboptimal care. Similarly, the youth may experience challenges accessing professional help in mental health (Damian et al., 2018). Another major barrier identified by youths is parental consent to participate in mental health research. Findings suggest that many parents lack an understanding or adolescent mental health and hence the youths may be reluctant to seek consent (Cavazos-Rehg et al., 2020). Failure to participate in research projects also leads to poor reliability of EBP which can be used to advance interventions for 5 mental wellness. Resources, access, and lack of knowledge are some of the common barriers to providing quality mental health services to adolescents. Elsewhere, current literature has not tackled much on nursing standards and policies for mental health among adolescents. However, the experiences of nurses in delivering mental health care to the age group have been highlighted. Jönsson et al. (2017) found that nurses in schools had a state of worriedness due to their patients’ mental wellness. However, providing mental health services was an opportunity for professional and personal development and brought satisfaction in the experience of making a difference in the lives of young people (Jönsson et al., 2017). Current policies and standards allowing school nurses to provide mental health services to adolescents, therefore, facilitate a framework for understanding challenges that young people face and the need for improvements in mental health services. Nurses are also central to the policy-making process and should be involved in creating policies to improve adolescence mental health outcomes. The nurse’s role has traditionally been to implement policies and provide care to patients. However, as identified by Salmond and Echevarria (2017), EBP is increasingly shifting nursing roles to policymaking especially through being fully-contributing members of interprofessional teams. This role of the nurse is significant in determining necessary interventions and how they should be implemented to make the most impact and bring the most benefit to the target population. Additionally, nurses are now increasingly serving on the governing boards of healthcare organizations and this increases their ability to affect change and engage in policymaking (Cleveland et al., 2019). This shift in nursing role provides an opportunity for nurses to lead change in implementing mental health interventions among adolescents. 6 Lastly, various nursing and healthcare theoretical frameworks may be used in promoting nurses’ role in adolescents’ mental health. Comprehensive assessment and care theoretical framework is one of the approaches that has been identified in the literature. Mental health services in schools could be transformed to ensure comprehensive patient assessment and interventions (Cavioni et al., 2020). This is especially because in the past, a reactive approach has been largely deployed as opposed to effecting proactive assessments. Nurses can be trained to promote social and emotional learning in schools and also assess and identify behavioral problem risks early to prevent behavioral problems (Cavioni et al., 2020). Generally, a comprehensive assessment and treatment approach should be deployed in schools to improve mental health outcomes among adolescents. Standards and Policies Nursing standards and policies influence the quality and impact of healthcare focusing on the mental health outcomes of adolescents. One major policy affecting these outcomes is the implementation of integrated care models in the past decade. Most integrated care models were created under the Affordable Care Act (ACA) and use psychiatric mental health nurses as well as advanced practice nurses as health providers and managers (Delaney et al., 2018). This policy has enabled nurses to b on the forefront of providing mental healthcare to adolescents and other populations. Therefore, the policies implementing integrated health models in different healthcare settings promote nurse participation in enhancing the mental health of adolescents and others as well. On the other hand, the scope of practice in nursing is significantly limited due to state and federal policies. One of the effective means of providing access to mental healthcare is through 7 telehealth services. However, current telehealth use policies restrict majority of nurses from using the technology to reach their clients (Gajarawala & Pelkowski, 2021). These policies limit reimbursement-related to telehealth services to only some professionals, mainly physicians and advanced practice nurses. This reduces the scope of practice in restricting nurse use of technology to assess and provide mental health services to adolescents. While some policies and standards enhance nurses’ ability to provide care, others are limiting factors and barriers to effective mental healthcare services to the target population. Leadership Strategies Leadership must play a central role in enhancing the mental health of adolescents. The main role of leadership in this project is facilitating care and education on mental wellness. A great leader is an educator and uses their influence to enable others’ success through knowledge and guidance (Barr & Dowding, 2019). Therefore, facilitation and ensuring adequate access to knowledge for nurses and the target population is an important leadership approach. Leaders also motivate their followers by creating a vision and working towards its completion. Creating clear intervention goals and a pathway to achieve them will be another leadership strategy to use. Lastly, collaboration, communication, and change management are also essential for successful EBP and will be used in this project. The main strategy for collaboration and communication will be initiating an interprofessional team for the change. Moreover, in change management, leaders will ensure stakeholder buy-in by providing proper education and involving them in designing and implementing care interventions (Barr & Dowding, 2019). Collaboration and communication will be enabled in the interprofessional care context while change management will be made possible through stakeholder engagement. 8 Conclusion Nursing interventions to address mental health outcomes for adolescents are necessary and should be implemented in an EBP context. Mental health is a significant issue in this population group and should be addressed to reduce the rates of teenage depression. Nurses are central to such EBP projects and should be involved in planning and implementing policies. Current literature supports the role of nurses and EBP in addressing the health condition.
