Introduction
The United States Food and Drugs Administration (FDA) (n.d) defines medication error
as “any preventable event that may cause or lead to inappropriate medication use or patient harm
while the medication is in the control of the healthcare professional, patient or consumer.” There
are several organizations that are set to review reports concerning medication errors. Some of
them include; Center for Drug Evaluation and Research (CDER) and the Division of Medication
Error Prevention and Analysis (DMEPA) (FDA, n.d). The formation of the bodies shows how
the issue has become common and controversial in the healthcare system.
Purpose
This project will explain the issue of medication error in the United States, its causes,
what the government and the healthcare system is doing to mitigate the issue and provide
