Be sure to provide a link to the program website. Post your response to the prompt in your group forum by the deadline. Primary posts should be 300-400 words long. Grossmont Cuyamaca Community College District Nutrition Worksheet
Part 1- Current Trends
Complete the following "two quizzes":
Healthy Eating Quiz (Links to an external site.)
This involves a series of 15 questions, which will provide a basic picture of your diet.
Directions:
You will read each statement and based on the statement you will select either "Always", "Sometimes", or "Never".
Once you have finished, hit the "Get My Score!" button. Your result will immediately appear to the right of the button.
Life Expectancy Calculator (Links to an external site.)
This quiz will estimate your life expectancy based on a series of questions from the following categories: personal, lifestyle, nutrition, medical, family. Please note that this is only an estimate and that it cannot truly predict your life expectancy.
Directions:
Click the yellow, underlined text that says "Take the Calculator".
You will need to input some personal information. I would recommend not including your exact birth date or zip code (you can use the school's zip code, which is 92019). Once you have filled out this information, click the "Proceed to Calculator" button.
Complete the questions (there are 5 pages total, based on the aforementioned categories)
Once you have completed the questions, the site will calculate an estimated life expectancy. It will ask you for your email and a password for the site. Again, I would recommend a junk email (although myself and other instructors have never experienced issues with this website).
Answer the following questions:
In regard to the Healthy Eating Quiz:
What score did you receive?
What do you think impacted (whether positively or negatively) your score? Explain.
In regard to the Life Expectancy Quiz:
How long did the test say you were expected to live?
What are two things you are currently doing that is shortening your life expectancy?
What are two things that you can do to lengthen your life expectancy?

Which vitamins and minerals were you shown to be deficient in?
Did any of these results come to a surprise to you or were you expecting these results?
Part 3- Tracking
Food Log:
You will complete a 3-day food log. We are doing 3 days in order to get a basic understanding of our eating trends, although you are welcome to track for more days in order to have more robust data.. You can choose which consecutive days you complete the log; however, I would strongly encourage you to do the log between Monday and Friday since we are generally more consistent in our eating habits during these days (example- parties and other celebrations are more likely to occur on weekends).
You will be responsible for recording a list of the foods/beverages from each meal/snack, as well as their serving sizes and nutrients (carbohydrates, fat, and protein levels, along with selected vitamins and minerals).
