Community Assessment The city of Pembroke pines has a population of approximately 171,178 .It is the second largest city in Broward County. It is located in South Florida between Miami Dade and Palm Beach.Pembroke Pines is bordered by the Everglades west,1-75 central and East the Florida Turnpike.A diverse population lives in Pembroke Pines. The demographic data ethnic groups are predominantly white 60.4%,hispanics 45% and blacks 22.3%.(United States Census Bureau,2021). This community has two hospital in close proximity to Pembroke Pines, several support group inclusive Planned Parenthood of South, East and North Florida and Womens, Infant and Children(WIC) support group. Planned Parenhood is an organization that defend womens reproductive rights and provide critical health care services in Pembroke Pines such as gyneocological examination,birth control, abortions and other primary, secondary and tertiary intervention services. Similarly,WIC is a program that support pregnant women and pediatric population who are at risk for impaired nutrition. The median household income is $68,0683(United States Census Bureau,2022) ● According to statistics obtained by Cleveland Clinic(2016),Florida Department of Health highlighted that Broward County has significant high incidence of Sexually transmitted infection inclusive HIV, Congenital Syphilis, AIDS, gonorrhea and Chlamidia which Pembroke pines is a part of that county(p.51) Literature Review Pembroke Pines has over 800,000 individuals living with mental illness(Schumacher,2022) Geography of Pembroke Pines According to the United States Census Bureau the geography of Pembroke Pines population is 4,671.9 square miles.It has 28 Parks .While conducting the windshield survey the memorial Pembroke Pines Hospital was observed .Several educational institutions including charter schools were observed along with food establishment that can be easily accessed. b. Aggregate (Target) Population ● Population #1: Low-income teens, women, and men seeking sex education, reproductive health care, and mental health ● Population #2: Low income pregnant and postpartum women, infants, and children seeking nutrition education and assistance Aggregate (Target) Population Population #1: Population #2: Gatekeepers and Key Informants • Identifies gatekeepers or key informants who will assist the community health nurse in gaining access to the population of interest. c. Community Health Diagnoses • Includes two community health diagnoses using the data from the community assessment. • Includes one wellness diagnosis. • Diagnoses are listed in the order of priority justified by the data findings and analysis. • The diagnoses consist of four components: the identification of the health problem or risk, the affected aggregate, the etiological statement, and the support for the diagnosis (Nies, 2019, p. 102). d. Plan for Priority Diagnosis • Includes a minimum of 1 short‐term and 1 long‐term goal for identified priority diagnosis. • Goals relate to the identified priority diagnosis. • Goals follow the SMART format: specific, measurable, attainable, realistic, and timed. • Explains how the plan allows for client involvement. • Explains how the plan advances the knowledge of members of the community. e.
