Educating Patient through mhealth App
Patient Scenario
It was a busy evening in the hospital when I met a young couple. The adult male was
heavily overweight and newly diagnosed with diabetes. The wife was worried about his
condition because his blood glucose level record was not good during the last two weeks. He was
diagnosed with diabetes type II the previous month, and it was observed that he was not taking
any medication since then. They were trying to control the condition by themselves. I examined
the patient, and their blood glucose level was 280 mm/dl. It was one of the alarming figures in
the fasting condition. According to Healthy People 2030, 30 million Americans have diabetes
type II, and the goal is to reduce the number of diabetics and manage diabetes type II. The
management of diabetes II is extremely important because it is observed that diabetes should be
ultimately treated because there is a risk of cardiovascular disorder and others (Deborah J
Wexler, 2020). the overweight condition and being obese are included as the risk factors for
diabetes type II. The patients need to be educated about all the risk factors, and diabetes should
be managed properly.
Patient Education/Teaching Plan
The wife was the husband's caretaker, and they were enjoying their honeymoon period
when it was diagnosed that the patient had diabetes. She was highly worried because they were
trying to conceive as well. I needed to tackle the patient and his wife, and it was necessary to
educate them. The first thing that I had to inform was not to panic. I handled the issue calmly
because I deal with many people suffering from diabetes, and I knew that the lady was sensitive
because she was concerned about the patient's health. The first thing that the couple should
understand is that diabetes type II has no cure, but it can only be managed through care.

The management of diabetes type II is essential because it affects genetics and behaviors.
If the behaviors and attitudes of patients are not healthy in life, it means that there will be
challenging to deal with diabetes. After getting over the nervousness, the patient's first need to
understand is lifestyle modification. Diabetes has to go with life, and it can only be managed.
Patient education remains incomplete if they are not informed of the risk factors. The risk factors
for diabetes type II are cardiovascular arrest, metabolic syndrome, and many others. The patients
need to avoid the situation and keep a check on their blood glucose levels.
The next important thing is the behavioral change that the patient needs to bring into the
lifestyle. The patient was overweight, and it seems to be the root cause of his diabetes. The
overweight condition was the most important thing to be minimized. The couple said that the
patient had gained enough pounds in the last few months. I educated them to lose weight and
ensured that diabetes could easily be controlled through lifestyle changes.
The lifestyle modification is easy, but the patients need to be consistent with the routine.
The exercise must be part of the routine, and a morning walk for thirty minutes is recommended
for the patient. Physical activity is necessary for the diabetes patient because obesity and a
sedentary lifestyle are the reasons for diabetes. It is observed that the patient goes to the office,
and mostly, he had to work in the office, and the lifestyle was sedentary. So, physical exercise
was a necessary change in the lifestyle.
The next thing was the change in eating habits, and the couple already knew that the
patient had to avoid the sugary food items. I further educated them that sugary items alone are
not prevented, but the patient needs to overcome the habit of eating high caloric, fatty, and transfat-based foods. The couple consumed a lot of high-caloric and junk food in the last few months.
I recommended that homemade foods should be excessively used. The fruits and vegetables

should be made part of the routine. Fruits and other food items with a high glycemic index
should be avoided. The foods rich in nutrients and low in calories should be consumed. Whole
cereals and foods with fiber must be included in the daily routine. It was observed that the wife
was highly conscious, and she committed to taking proper care of her husband.
As far as the medication was concerned, I suggested not inserting the artificial insulin at
the start because the condition could be managed easily through lifestyle change. I asked them to
record the blood sugar level for a month to observe the change. It will help them to remain
consistent throughout the journey.
Description of mHealth app
DM: Fit™ is a mhealth app that helps people with diabetes to manage their exercise and
nutritional program. The patient was overweight, and people with diabetes already needed the
proper nutrition to keep their blood glucose levels controlled, so the DM: Fit™ is one of the
appropriate applications for the users (DM: Fit, 2022). The intended audience is the diabetics,
and it can also be used for the overweight people to keep their nutrition in balance. It is easily
available on android mobile phones and can be easily downloaded. The patients can easily
download from the play store and get the benefits. There are several benefits of mobile
application, and it is appropriate for the current patient.
I suggested that the patient install the health app on his mobile and set his targets. The
application will help to know the timings and intensity of exercise. It will allow him to set the
nutritional goals and help his wife decide on the meals. It is one of the appropriate applications
for the patient. The couple was pleased to visit me, and they left with the hope of managing
diabetes as early as possible.

Purpose
The purpose of this assignment is to demonstrate the skills of the professional nurse as
an educator. You are to prepare a patient scenario based on the required assigned
topic. You will choose an already developed and reliable mHealth app to use in the
education of your patient.
Based on your accepted Milestone 1 assignment, use the information to identify three
teaching areas to improve patient outcomes. You will also determine three ways to
evaluate the success of the patient's use of the mHealth app. You will need to provide
the mHealth app reference in APA format.
Required Assignment Topic
Note: You will use the same scenario you developed and same mHealth app you
identified in Week 2.
Requirements and Guidelines
