The purpose of this case study is to engage in the development and progression of the Information Governance (IG) principles (accountability, transparency, integrity, compliance, protection: retention & disposition, and availability) as they are applied to mobile applications

The following case study outlines considerations and challenges of integrating patient generated data (originating in a mobile app) into the health record.

II. Organizational Description:

An endocrinology practice is encouraging diabetic patients to use a mobile app that tracks blood glucose levels. The purpose is to integrate the data into the electronic record.

III. IG Program Description:

The practice manager, an HIM credentialed professional, has been identified as the lead on this initiative. Concerned about the privacy, security, and integrity of health information, the practice manager understands the necessity for policies and procedures to protect patient information and the integrity of the record. IG principles listed below should be considered when developing an IG program.

IV. Prior State Analysis:

1. Until the results of the mobile app pilot project were analyzed in May 2016, the practice was not encouraging the use of a mobile app for disease management. No patient generated information was considered in clinical decision making.

V. Information based on the IG Impact/Challenges:

The data generated are glucose levels retrieved from an at home medical device. The device then transmits the blood glucose data to the vendor's proprietary cloud service. The vendor’s cloud service has an application program interface (API) that makes this blood glucose data accessible to third party systems. The endocrinology group would need to partner with a regional or state level health information exchange (HIE) to begin gathering data from the glucose monitoring vendor. The HIE would then call into the vendor's cloud and transmit the data into an internal HIE database. The patients demographic information would be utilized to connect the new glucose data to existing HIE records associated with the patient.

Note: Data in motion must be encrypted. This would need to be facilitated by the entity calling the API, which would be the HIE. The HIE would then transform this glucose data into a standardized format. This is important because many vendors collect glucose data and store it in different formats. The HIE must standardize all of these data into a common format. The Endocrinology group would then use their EHR to call into the HIE to transmit data, either directly into the EHR, or through a web application viewable by the physicians.

Note: The connection from the HIE to the Endocrinology group would be encrypted. There would also be a business associate agreement (BAA) in place to comply with HIPAA.

Data would not be created until the physician validates the data and allows its import into the legal health record.

Nursing staff would monitor the inbox for the EHR. They would review the glucose data and forward to the physician for a signature if it is valid.

The HIE is responsible for the storage of the data before it gets to the provider. HIE messages entering the EHR would be the responsibility of the Endocrinology group.

Processes should be created.

Individual providers would not want to enter into BAA’s with every single mobile application. That is why it is important the HIE take this responsibility to retrieve the data first. Then the BAA is signed between the HIE and Endocrinology group.

Maintain data indefinitely if possible. Purging data is no longer necessary with digital storage. At minimum follow HIPAA or state law.

TASKS:

In order to realize the goal of integration of Patient Generated Health Data into the EHR:

  • Identify the consideration of the impact and challenges with integrating PGHD into the EHR (detail and explain).
  • Identify the benefits of integrating PGHD into the EHR (detail and explain)
  • Identify any workflow, job description modifications, and development of what type of policy and procedure would be necessary for a successful implementation (you will not be developing the policies and procedures, rather identifying the ones that will need to be created including the overall gist of the policy/procedure).
