Initial Treatment Plan: Eliza

Part 1: Intake
Eliza a college student aged 18 years old in her freshman year got into trouble with the
school authorities. She is of Caucasian descent with a lean body and of average height studying
engineering at the school. Her parent’s home is approximately two hours away from the college,
and thus Eliza can visit whenever she likes. Eliza went for a counseling following an event that
took place in her dorm room a few days before. Eliza and friends in her dorm room were caught
with alcohol which is illegal since her school is an alcohol-free zone. She admitted to having
partaken in alcohol but because her friends were also taking alcohol. She also claimed having felt
tipsy or using her word buzzed while her friends were drunk. The intake form helps in
establishing a few elements that are necessary for treatment of Eliza and also identification of
any problems she might be having subconsciously. Additionally, the form contained all the
information provided by the client in the level one cross-cutting symptom measure.
A level one cross-cutting symptom measure was taken by Eliza to help in identifying any
issues or problems that could have explained her behavior recently. The cross-cutting
examination was helpful in assessing whether Eliza suffered various symptoms such as
depression, mania, anger, somatic symptoms. Suicidal ideations, personality functioning, sleep
problems, substance use, repetitive thoughts, behavior dissociation, anxiety, and psychosis. The
scoring and interpretation of the measure form was done, and a few factors brought forth. The
primary purpose of the Level 01 Cross-Cutting Measure (CCM-1) was to help in identifying the
underlying problems that will help in explaining her behavior...
