how do i change two data points into a slope intercept?

i need to know how to make two data points into slope intercept form. two data points would be (10,550) and (12,650).

two point are (10,550) and (12,650).

slop equation

Y-Y1 = m (X-X1)

m=(X2 - X1)/(Y2-Y1)

m=(650-10)/(12-550)

m=640/(-538)= -1.18

Slop intercept form

Y-550= -1.18(X-10)

Y+1.18X =561.8

the new points i have is 12,192 and 18,288 this is what i got 

288-192= 96

18-12= 6

96/6= 16 

y=16x+b

192=16(12)+b

192=192+b

-192     -192

y=192 - 192  ( is this answer correct)
