Article: The effectiveness of information technology- supported shared care for patients with
chronic disease: A systematic review
Introduction
The author provides information related to the way in which healthcare facilities have
embraced the use of technology as a way of sharing information related to the patients with
chronic diseases. Most hospitals have adopted the use of technology as a way through which they
store data about patients and which they share within departments to ensure that the patient
receives quality care. However, chronic diseases have continued increasing a reason why there is
the need to understand the best ways through which the facilities can share information to
manage them effectively (Kooij, Groen & van Harten, 2017). Technology has made the process
of offering treatment to the chronic patients easy and effective where they can handle the
conditions and ensure that they have better lives in the long term. The authors argue that in the
case of chronic illnesses treatment, more than one health practitioner is involved. As a result,
there is the need to provide each of them with information that explains the health needs of a
patient a way through which they receive quality care.
Objectives
The aim and objectives of the authors are to ensure that they have reviewed the literature
which explains the effectiveness of IT-supported care and interventions that would make it easy
to deal and handle cases of chronic illnesses. This will focus on the responses applied in the
management of chronic diseases regarding the professionals, processes and the clinical as well as
financial outcomes when utilizing technology (Kooij, Groen & van Harten, 2017). On the other

hand, the authors focused on gathering information that would assist the healthcare practitioners
in identifying the best type of technology which they could use to ensure that patients recover
from the chronic diseases through proper management. Therefore, acquisition of IT with the
characteristics which would support the provision of care to the patients with chronic illnesses
remains the focus of the authors as they identify some of the types of information technology. On
the other hand, the authors seek to help the healthcare facilities where they recognize the
different methods and ways through which technology will aid in taking care of the patients.
Literature review
The authors provide a literature review where they seek to bring to light some of the
previously done research activities which explain the importance of technology in healthcare
facilities. In this case, they focus on management of chronic diseases and instances where the
application of technology played an essential role in ensuring that there was the success in the
long term. For instance, the authors state that cancer is one of the conditions that has required the
utilization of technology since the patients have to go through a variety of operations where they
will receive different types of care (Kooij, Groen & van Harten, 2017). The coordination of care
between multiple professionals has called for the need to understand the impact of using
technology and ways through which it can aid in transferring information from one activity to the
other. The authors have given detailed information related to research done in the past that has
shown the success of implementing technology in handling and managing chronic illnesses.
They also explain that the effectiveness of technology based on previously done research
depends on the coordination of the healthcare services providers.
