ASSESSMENT AN DIAGNOSIS

Definitions
a) Screening
In medicine, screening is the process of examining a population to identify the likelihood
of the presence of a yet-undiagnosed disease on persons without any signs or symptoms. It is
carried out on individuals with the unrecognized symptomatic disease who are seemingly in
good health to direct preventive action or allow for further investigation and management to
reduce mortality and related suffering from the disease.
b) Assessment
Assessment is the evaluation of an individual's health status by carrying out a physical
exam after looking through their health history to detect the presence of diseases in otherwise
healthy people. A health assessment aims to determine where an individual lies on the health
continuum to get a guide on how to approach the situation and treat the individual (Chassin et al.
2003)
c) Treatment plan
This is a documentation tool built around the complications and problems the patient
brings into treatment, and it's considered critical for the implementation of well-rounded health
care. A treatment plan contains a detailed list of the patient's physical, behavioral and emotional
problems and it informs the staff what brings the patient to the healthcare.
