DESCRIPTION

ADVERSE EVENT
An adverse event simply refers to the medical errors that may arise when a
patient is being diagnosed/treated. The errors can either be intended or
unintended and as a result it may lead to serious injury, death, prolonged illness
or any other chronic conditions to the patient. The chosen adverse error in this
paper is patient falls. Patient falls occurs when the patient accidentally descents
to the floor as a result of either patient’s personal factors or environmental
factors. The personal factors that can cause patient falls are things such as
chronic medical conditions, fragile muscles or crutches for the patients that need
walking support. On the other hand, environmental factors refer to the things
around the hospital premises that can result to the fall such as dustbins or beds
inappropriately positioned. Aside from the two factors, also the care related
factors may cause patient falls such as a nurse not responding promptly to
emergency calls from the admitted patients.
Scenario
A nursing note entailed that a 75 year-old patient was found lying on the floor at
the door when she entered the sickroom of the hospital. It is alleged that both the
physician and nurse assigned to attend to the patient were temporarily missing at
the time of the incident. The nurse was passing by and heard a shattering sound
as a result of the patient falling. And so when she checked in, the patient was
lying by the door. The role of the assigned nurse in this case was to stand by as
the physician had gone away in search of other medical interventions.
HISTORICAL
BACKGROUND

It is estimated that about 700,000 to a million patient falls are reported annually
in the United States hospitals. Correspondingly, it's estimated that about 30-51%
of the patient falls often result to the patient getting injured (Butcher, 2013). In
2008, the Centers for Medicare & Medicaid Services decided to cease paying for
