Location of incident Northwest corner of campus Northeast entrance of WC Morris Description Chewing Tobacco located near Shorty In chipper expulsion blood and guts where found Suspect wearing plaid shirt and blue jeans, working boots not steel toe At around 11 am the safe team arrived to a taped off scene. The crew member in question was located waist deep in side the wood chipper, deceased. Upon arrival there was one supervisor on site, four crew members, and the husband of the deceased. The wood chipper was no longer running and all crew members were stepped away from the chipper. The whole scene was on a flat grade. The weather was 43 degrees fahrenheit and cloudy but there was no rain or moisture that could have been a causal factor. The deceased crew member was wearing boots, blue jeans, and a plaid shirt. About six feet away from the expelling port wood debra and chunks of Shorty. Analysis (what unsafe acts or conditions contributed to the incident) There was multiple potential causal factors. All of the crew members stated Shorty was not wearing any PPE. His clothing was loose fitting and no steel toes. The machine had no tire chocks. With using a Wood Chipper a push board should be used to prevent reaching, there was no push board in sight. All employees stated they don’t use them. For the start of the day there is not any pre-work check off sheet to ensure everything works properly and all safety procedures are put into place. Crew members believed he was using chewing tobacco for the first time on scene. This will leave the user ill fit to function properly. The new found chewing tobacco use was possibly caused by a peer pressure to fit in. The employee had only been employed by the company for 3 months and may have had inadequate training. Based off of information from the husband, Shorty had also been laid off from work for a year and had a lot of pressure to bring home a paycheck. Corrective action(s) (list correction action items, responsible person, scheduled completion date) Supervisors will be retrained on their duties, emphasizing in enforcement of all safety procedures and their involvement in each scene. This should be done within the next week and the head of the training division should initiate this. Long term employees will be reminded that all safety precautions will be used and that supervisors are being retrained. Communications should be incharge of this and it will be sent out immediately after the supervisors retraining New Hire Training will be re-evaluated and considered for re-wrote, to see if there are any ways the program could be improved. BACKGROUND/PRECAUTIONS
The case in focus is that of a victim of the wood chipper killing. The person operating the wood
chipper did not exercise the necessary precautions while operating 5he machine and that is how
they ended up dying from the crushing by the wood chipper.
The precautionary measures when using the wood chipper are many. The first precaution is the
wearing of protective and fitting clothing when operating the machine. With the proper an
protective clothing necessary, the individual running the device will be able to protect themselves
from minor injuries and will be in a position to stop the machine in case of an incidence. The next
precautionary measure is to read and understand the instructions of the device and in the machine
manual. The instructions in the manual allow you to have an overview of how to operate the
machine and make you understand the necessary steps of operation. The manual as well will enable
you to follow the precautions to take in the activities of the apparatus.
Before taking up the operation of a wood chopper one also needs to get proper training from a
qualified individual on how to operate the machine and make sure they understand every step of
the process. The practice can only happen at the pace of the learner. The pace of the learner means
