- How will you proceed with Mr. Jones?
- Mr. Jones will be treated like a new patient with a focused assessment on his risk for falls. A thorough history of the present illness, a review of systems, and then a complete physical assessment will be appropriate. Fall is one of the most prevalent diagnoses for older adults. A fall is defined as an event where an individual descends onto the ground without the purposeful intention of it. Falling is correlated with mortality and a decline in function (Appeadu & Bordoni, 2023).
- What diagnostics and screening will you perform?
- According to the United States Preventive Service Task Force (ASPSTF), his age of 67 years old, places him on the radar for screening for prostate cancer, colorectal cancer, hypertension, depression, fall prevention, and suicide risk, and diabetes (Search Results | United States Preventive Services Taskforce, n.d.). His wife is present, and they both appear to be reliable historians.
Differential diagnoses to consider are orthostatic hypotension, dizziness, and alcohol associated with intoxication.
What will potentially be part of your treatment plan?
- The treatment plan for Mr. Jones is to screen him for hypertension, hypotension, and diabetes, assess his ASCVD risk factors, and order labs: CBC, CMP, Fasting Lipid panel and Hgb A1c, and a liver panel.

Treatment options available include behavioral, medications, support groups, and healthcare professionals. Drugs such as naltrexone, acamprosate, and disulfiram are available for treatment under the proper clinician guidance (Treatment for Alcohol Problems: Finding and Getting Help | National Institute on Alcohol Abuse and Alcoholism (NIAAA), n.d.). Providing a sincere understanding and concern for their well-being is critical in starting treatment.
What specific resources in your community (city or region) will you refer him to?
Local resources that have great reviews are TouchStone Recovery Center in Fresno, California. The program vouches to provide individual care for each individual and accepts certain insurance as well (Team, 2024). Another rehab center that utilizes evidence-based dual diagnosis in Fresno and is certified by the Joint Commission is My Time Recovery. Therapies at the center include fitness, yoga, sound, and adventure. The facilities house individuals throughout the treatment program (My Time Recovery, 2024)
