Significance
over 34.2 million patients in the U.S. are living
with diabetes. The DRIF in 2022 observed that
the increasing number of diabetic cases in the
nation is a case of alarm as it creates an urgent
need to prepare primary medical care to
respond to the related complications (3)
Increase access to
food> healthy foods (5)
Provide
education on
ways to prevent
hyperglycemia
and
hypoglycemia
Diet literacy
(5)
Educate population to
address knowledge
deficit (5)
Lack of education on how to
prevent diabetes among
vulnerable population>
persons aged 40 years and
above (2)
There are serious
complications of Type 2
Diabetes in Primary
Care Settings (1)
Prevention strategy
Education program (4)
Aims of the Project
• To investigate the impact of an
education program in managing diabetes
for venerable patients aged 40 and
above.
• To investigate how an education
program helps prevent complications
related to diabetes within six months of
diagnosis for vulnerable patients aged 40
and above.
Components of Education
Program
▪ Online facilitated lifestyle and diet
management classes for patients
with access to the internet.
▪ Workshops and training on lifestyle
and diet management classes for
patients without access to the
internet
▪ Training guide to be issued to the
target group.
Outcomes:
▪
Provide
education on
alternative
sources of sugar
Educate persons aged
40 years and above on
how to manage the
intake of food and
develop a healthy
lifestyle (4)
Impacts:
▪ Decreased prevalence of severe
diabetes among persons aged 40
years and above.
▪ Reduce new cases of diabetes
among target population. (4)
▪
Persons aged 40 years and
above will develop
healthy lifestyle.
Target population will
develop diet management
skills. (4)
1. Lack of proper
management of type 2
diabetes (T2DM) leads to
serious complications
(Nikpour et al., 2022).
2. Lack of health education
increases the risk of T2DM
among persons aged 40
years and above (Garg et
al., 2019).
3. Hyperglycemia and
hypoglycemia are the most
prevalent complications
that affect vulnerable
patients aged 40 and above
with diabetes within the
first six months (Diabetes
Research Institute
Foundation, 2022).
Diabetes is a serious
metabolic disorder affecting
millions of people across
the globe (Dhatariya et al.,
2020).
4. Health education programs
for diabetes increase health
outcomes by preventing and
managing T2DM (Garg et
al., 2019).
5. Dietary literacy helps to
address barriers hindering
patients from eating healthy
foods (Bross et al., 2021).
