The purpose of this organizational development needs survey is to provide a tool for
performing a crucial skills evaluation in the organization. Using this framework will help the
organization establish if necessary skills gaps are impacting its performance and recognize the
skills set that will require upgrading (Fink, 2011). The survey questions will ask leaders to evaluate
their team, team members, and to rate themselves.

1. Generally, how successfully did your team cooperate on the organization’s project?
