  • Summarize community health portrait including the strengths and risk factors.
  • Identify at least three desired personal behavioral and health outcomes.
  • Identify at least two desired family health goals.
  • Recommend specific strategies for realistically achieving identified health goals. Explain how appropriate community resources can be utilized and how those goals will be met.
  • Explain how, if implemented, these goals will benefit you, your current family, and future generations.

At the end of your plan, attach these three parts: 1. Family Portrait (1–2 pages), 2. Community Portrait (1–2 pages), 3. Health Promotion Plan (5–7 pages).

PART 1: FAMILY PORTRAIT
The health assessment is basically a plan of care involving an interactive, deliberative and
systematic process used by nurses to synthesize, analyze and validate the information they
collect from either communities or individuals and even their families based on their health
history and by taking and by performing a physical exam. After performing a health assessment
on myself and my immediate family, I found out that I was living a healthy life as opposed to my
immediate family (NCI, 2018). My personal assessment showed that I was adopting good and
healthy living habits and I did not suffer from any health risk factor but my immediate family did
the opposite. Some of the health habits I and my immediate family meaning my mother, father
and my two sisters and brother included eating healthy breakfast every morning, exercising for
twenty minutes on a daily basis, getting enough sleep, drinking a lot of water, eating a lot of
vegetables and fruits, frequent medical checkups, making social connections, taking up a hobby
for example me, my father and my brother we love watching football while my mother and sister
both like playing cards, and practice dental hygiene as well as taking a walk on a daily basis.
However, apart from doing all these healthy habits, there are still various health risk
factors pounding me and my immediate family. Those impounding me include depression and
stress after breaking up with a fiancée. Those impounding my immediate family, however,
include alcoholism and high blood pressure ailing my dad, obesity and tobacco smoking as well
as diabetes mellitus ailing my brother and my mother suffers from hypertension just like my
father. My mother also suffers from a heart condition which has brought about the hypertension
problem (NCI, 2018). I did not find health risk factor ailing my sister. Generally, the family is
involved in a lot of healthy habits but there are some of the risk factors which can be managed by
the health habits and this is why we are trying as much as we can to do so. Levels of colorectal

cancer risk and diabetes were high for my father, mother and my brother while I and my sister
recorded a very low risk of the same (NCI, 2018).
PART 2: COMMUNITY PORTRAIT
A windshield survey is basically a systematic observation that is made from a moving car
(Nathan, 2018). Upon performing a windshield survey across my community and recording some
of the resources and characteristics including schools, health care, social or human service
agencies, types of transportation, types and condition of housing, recreation facilities, ethnicities
and religions, and shopping as well as restaurants/bars, there are some conclusions I drew. First
of all the community is based in a posh neighborhood. There are two elementary schools and one
high school. There is only one high-class hospital which seems to offer high-quality health
