My community is Logan County, New Jersey State, USA. The Grant topic is Type 2 Diabetes.

Grant Goal: Decrease breast cancer mortality in selected populations. Community goal: Decrease breast cancer mortality in Highlandtown and Park Heights communities. By the end of the sixth month, 100 women in the program will be able to perform BSE with 100% accuracy. By the end of the sixth month, 50% of women will demonstrate an improved attitude about breast health. Create monthly breast health sessions at local churches. Session will follow last church service on Sundays. Meet with church officials to schedule these AND to have them advertise the sessions to parishioners. Create all material for sessions. Session topics 1. Overall women’s health and role of women in the family; 2. Women’s breast health-facts from public health worker and discussion of BSE, yearly exams and mammography; 3. Female breast cancer survivors (preferably from given community-OR same race/age of the population) and practicing of BSE; 4 creation of marketing tools so they can market to other women in the community; 5 celebration families will attend one session/women in the group another. Families to learn importance of encouraging breast health, women to reinforce role of women in family, following these sessions...luncheon/cookout to celebrate women’s health and family. SAME 1. Do a survey that asks if they can do it. 2. Give them the steps and have them put them in order 3. Observe them with a check-off list 1. Give a pre/post likert scale asking questions like... How important is BSE. How important do you think a yearly breast exam is? How important do you think it is to get mammography?
