  1. Reassess your treatment plan diagnoses, goals, and objectives based on the new information provided.
  2. Fill out and submit a new treatment plan evidencing the changes made in treatment utilizing the treatment plan template provided.

CHANGES MADE ON ELIZA’S TREATMENT PLAN

2

Why the changes occurred.
The changes made on the treatment plan occurred because of the ineffectiveness of
the original treatment plan to achieve the desired results. The reason for this assertion is that
the original treatment plan adopted a mandatory counseling approach for the client as
opposed to instilling the willingness of the client’s engagement in the treatment plan (Cohen
et al., 2000). As such, the original treatment plan failed because the client was not a willing
participant in the program. Therefore, the new reviewed treatment plan would focus on
making the client a willing participant to the program in order to ensure effectiveness while
achieving the desired results. Furthermore, the original plan also failed because the counselor
bugged the client with several treatment measures for each of the problem she experienced.
In this new plan, the counselor would only use the best two techniques in addressing the
client’s issues highlighted under each category.
Effectiveness and Validity of the Treatment Plan.
The effectiveness and validity of the new treatment plan lies in the new approach
taken in handling the issue. In the first place, the new plan incorporated the willingness of the
client to participate in the counseling sessions as opposed to simply attending the counseling
sessions because she was required to do so by the schools’ authorities. Secondly, the
counselors cut down on the different intervention mechanisms that were supposed to be
applied in the intervention of each problem raised by the client. As such, this enabled the
client to achieve the maximum output expected from the counseling sessions. Furthermore,
the new plan also incorporated family members to ensure maximum effectiveness. While the

client attended her counseling sessions alone on a weekly basis, her parents were required to
accompany her for the counseling sessions every once a month for solidarity and support.
Adjusted to Address the Changes in the Situation.
The changes made on the new treatment plan are focused on ensuring that the client
gets the best results ...
