Considerations in the Transition from Independent Living to Nursing Facilities
Different scholars reaffirm the perceived difficulties experienced by both the patient and
their family or caregivers in the transition from living at home to living in a nursing home or
assisted living facility. Living at home allows for autonomy and independence which is lost in
the transition into long-term care where the concerned individual, now a resident at the facility,
depends on the staff and physicians for help in activities they previously performed by
themselves. The family, on the other hand, is bound to experience panic, shame at the thought of
committing their loved ones, guilt, and grief, and periods of second-guessing their decisions, as
well as worry regarding the new living conditions and adjustment for their loved ones.
Consequently, it is essential that both the individual about to undergo the transition and their
family or caregivers be prepared for the emotional change. This essay, using the case of John,
Alice, and their Father, discusses the factors to be considered when offering counseling to such
individuals and families while outlining the anticipated challenges and recommending the
services that the families should ensure are available in potential nursing facilities for him.
As the person responsible for counseling the family regarding a possible transition for
John’s father from living at home to residency in a nursing home, it is of essence that I consider
some factors when discussing this with the family. The first factor is the patient’s condition and
its implication on his overall functioning. John’s father has Alzheimer’s disease, the most
common type of dementia which causes memory loss and affects the patient’s critical cognitive
