The use and adoption of electronic health record systems have promised a useful number of
benefits such as the provision of better care and reduction in the cost of healthcare. Despite its
benefit, unintended consequences have come up due to their improper use and designs.
There has been an increase in the recording of more amounts of bad data. New types of errors
have been significantly reported of which they compromise the quality of care and safety of
patients. An example was whereby the treatment of a patient with cancer was delayed by several
years due to a setting in the physicians EHR system that defaulted to an old normal pap smear
test result instead of the recent abnormal results. There has also been a case of a baby dying due
to a massive overdose of drugs as a result of an error in transcription when a handwritten order
was erroneously entered into the system. The errors obtained from inaccuracies and
inefficiencies in data entry have a negative effect on the decision-making process by the
clinicians.
Some of the errors can be prevented by the installation and activation of automated alert systems.
There should also be an active regulatory framework whose purpose is to monitor the safety of
the EHR system and ensure regular maintenance is carried out to avoid malfunction. They should
also ensure the system meets the specifications of the design, have no errors or bugs, and be built
based on dependent hardware and software platforms.

In conclusion, if the EHR systems are carefully monitored and maintained according to the
regulatory standards, the rate of inaccuracies will greatly decline, leading to better patient care
and safety.
