A 35-year-old male, Mr. NX, presents to your clinic today with complaints of back pain and “just not feeling good.” Regarding his back, he states that his back pain is a chronic condition that he has suffered with for about the last 10 years. He has not suffered any specific injury to his back. He denies weakness of the lower extremities, denies bowel or bladder changes or dysfunction, and denies radiation of pain to the lower extremities and no numbness or tingling of the lower extremities. He describes the pain as a constant dull ache and tightness across the low back.
He states he started a workout program about 3 weeks ago. He states he is working out with a friend who is a body builder. He states his friend suggested taking Creatine to help build muscle and Coenzyme Q10 as an antioxidant so he started those medications at the same time he began working out. He states he also takes Kava Kava for his anxiety and garlic to help lower his blood pressure.
His historical diagnoses, currently under control, are:
Type II diabetes since age 27
High blood pressure
Recurrent DVTs
His prescribed medications include:
Glyburide 3 mg daily with breakfast
Lisinopril 20 mg daily
Coumadin 5 mg daily

a) Introduction
Being at the age of 35, Mr NX could be experiencing back pains due to lack of exercise.
This is where his muscles could have become weak and unused. In addition, his anxiety problem
has increased the back pains he experiences. According to Rahimi et al. (2015), his anxiety leads
to expression of stress hormones by his body, and when this happens, the hormones cause the
muscles to contract. The tightened muscles of his back lead to his body being resilient to pain
which is prolonged, and in his case it has been ten years. His anxiety attacks could have brought
about back pain, muscle stiffness and tension thus leading to a prolonged painful back onset.
Since he is taking kava for his anxiety, it could lead to him having more anxiety issues instead of
reducing it. It also causes fatigue thus making him have a continuous onset of back strain which
results in pain. The kava could also be interacting with other drugs he is taking in heightening
the anxiety as well. Another reason for his back pain progression could have been brought about
by him taking up weightlifting. His back pains could have been brought about by improper
weightlifting (Coenen et al. (2014). He may be trying to keep up with his friend who has been
managing to lift weights better than him. Due to this he could have caused his muscles and lower
back to become strained since he is not used to the weight lifting exercises.
Weightlifting outwardly causes him to experience a painful contraction of the back
muscles which have been strained. In addition, his age could as well be another reason that he is
experiencing back pains. According to Mehrani et al. (2018) being between thirty and forty, he is
more prone to experience back pains and it could be coupled with him not doing exercises. Being
at this age, he could be using his time to relax thus remaining seated in most cases. When he
relaxes after work this could lead to him having the back pains if he sits a lot or does strenuous
