Abstract
Asthma affects children worldwide and is the most common chronic illness among children. Its
management is compromised by non-compliance to clinical practice guidelines. The purpose of
the study was to implement the Global Initiative for Asthma (GINA) outlines of practice and
evaluate effects of Asthma Control Test (ACT) and Peak Expiratory Flow (PEF) on
hospitalization rates and pulmonary referrals. A pre-intervention and Post-intervention
retrospective chart review were completed to evaluate the effectiveness of clinical practice
guidelines in a rural pediatric clinical setting. The outcomes showed an increase in
hospitalizations and number of pulmonary referrals. More poorly controlled pediatric patients
with asthma, the need for acute care intervention, pharmacological therapy revision, or
pulmonology referrals were identified through the use of the clinical practice guidelines.
Evidence-based practice is essential to promote optimal short- and long-term outcomes in
healthcare settings.
Literature review
In order to solve the clinical problem among pediatric patients with asthma, a model of evidencebased practice change was an appropriate systematic approach. PEF and ACT assessments are
useful in decreasing hospitalizations and pulmonology referrals. They are also part of the
recommended protocol for evaluation of asthma control (Bonagura, 2013).
The asthma control test (ACT) is a tool that is subjective and filled by the patient, parent or
clinical office staff. It is scored numerically to evaluate the effectiveness of control of asthma
based on the perception of the patient or the ...
