Introduction

Journal entry refers to the information that is documented in the medical field that helps
the healthcare provider get a better understanding of what is going on with their clients. It helps
in evaluating the client that has been counseled through providing descriptions of the client
devoid of violation of the Health Insurance Portability and Accountability Act (HIPAA)
regulations. It also helps in the identification of relevant history, prescriptions, and medical
information about the client. Journal entry also ensures that the diagnosis of the client is provided
using DSM-5 and at the same time provide explanations as to why cognitive behavioral therapy
is the effective option for the client. The journal entry will also explain the legal and ethical
implications that are related to the counseling session with the client (Lowenstein, 2011).
Documentation is very critical since it provides the relevant information in the event of legal
