Description
Using the population, intervention, comparison, outcomes, and time (PICOT) format, generate a clinical question for a problem or concern identified in health care. This can be patient specific, facility specific, or provider specific. Work with your preceptor to develop at least two research questions pertaining to your problem including the null hypotheses. Of the two research questions, choose the one that you and your preceptor agree on that will work best for your project.

Issue Identification
Acute care settings may contribute to patient falls as they experience changes in the
environment, placement of catheters and tubes, bed rest and treatments. These falls are usually
not fatal but they are preventable and thus an unnecessary cost to the healthcare sector. There
have been a series of reported patient fall incidents within our institution and an urgent need to
solve this problem has been identified. This paper’s PICOT question is: in an acute care setting,
how can implementing daily-staff huddles compare to after-staff huddles in reducing patient falls
in a 6-week period? The null hypothesis is: daily staff huddles have no impact on patient falls in
a 6-week period.
Problem Statement
Patient falls are a common occurrence that seems to be increasing rather than decreasing
despite comprehensive studies into the problem. An estimated one million patients in the United
States are making this a major patient safety problem. Majority of the time, patients fall by the
bedside or in the bathroom. The most common patients affected include those over the age of 65,
mentally ill patients and frail patients. However, all patients can be affected by medications,
