The Reason for Referral and Background Information
Purpose for Evaluation
This is the admission of Jane Smith a 14-year-old eighth grader. She was admitted after
exhibiting behavioral problems and a declining academic performance. She has also been absent
from class without excuse while also being recommended for in-school suspension three times so
far. Her grades are mostly C's and she shows no intention or interest in doing better. After careful
evaluations, the school psychiatrist recommended that several assessments be administered to
Jane. The Psychological Evaluation took place on 10/12/2013 at Johnson Middle School.
Background Information
To better understand the situation, several sources of data were used. The first source was
her family which provided information about her developmental progress, school history, family
history and behavior at home. From her parent's report, her developmental progress was reported
to be normal since it occurred within normal time ranges. Also, her parents stated that she could
be trusted and always got along well with her younger brother and younger children. However,
the parents also stated that she was easily frustrated and often seemed to be restless.
Furthermore, it was indicated that Jane was portraying an argumentative behavior and exhibited
signs of frustration. She had missed her curfew several times which the parents stated to have
been intentional. In addition, the parents stated that they had previously found her experimenting
smoking. Based on this information, her parents believe that her behavior will get out of control.
The second set of data was collected from her school records. In the current academic
year, the records showed that Jane had five excused absences due to illnesses and had no tardies
which were unexcused. She had also received an in-school suspension at least three times after

behavioral outbursts which occurred in the classroom. Also, her grades mostly consisted of C's
and failed in two of her classes despite her writing and reading skills being regarded as above
average for her age. Jane also seemed to work better with her teachers as compared to her peers.
Jane’s medical records further show that she was up to date with her required shots, her vision
and hearing testing were complete and her physical well-being was normal for her age.
