Implementation Plan
The project was about the prevention of new cases of diabetes in young people. Diabetes is a
lifestyle that can be prevented by changing lifestyle behavior. To avoid the occurrence of new
instances, the following action plan will be followed:
The first action plan is to provide the necessary education and awareness to young
people. Young people, especially adolescents, are not aware of the causes of diabetes. Most
of them do not know that they can reduce the risk of getting diabetes through eating a proper
diet and doing exercises (Babler & Strickland, 2016). Giving the necessary education and
creating awareness among the youth will help the child to abandon their harmful lifestyles
and adopt healthy lifestyles.
One of the harmful health practices is poor eating habits. The youth are used to eating
junk food, which puts them at risk of getting diabetes (WHO, 2016). We will measure the
change by given our target groups weekly goals. Every week, each group member will be
required to fill a form indicating what food they have eaten. They will also be required to
indicate whether the food is healthy or not. At the end of the week, we will analyze each
member of the group. This will help us to determine whether the youths are eating healthy.
From the results, we will be able to tell whether we have reached our improvement goal.
The second plan is to the provision of health promotion material. Many young people
have little or no knowledge about diabetes (Chao et al., 2016). We will distribute health
promotion materials in the form of pamphlets for the youth to read. The flyers will contain
different health information; for example, one booklet may have information about what
diabetes is, and another will talk about the causes. At the end of each topic, the participant
will be required to answer questions that will be marked and mark awarded. We will be able
to assess our success by looking at the number of participants. The health promotion
materials will also be distributed through social media groups. The participants will
