1. What is the primary problem and what is the underlying cause/pathophysiology of this problem?
This needs to include more than a definition of the disease and should get down to the cellular level.
What is happening in the body and why?
The primary problem is Covid 19, a respiratory pandemic disease.
Covid 19 is family of viruses that cause illnesses that are associated with common cold,
severe acute respiratory syndrome (SARS) and Middle East respiratory syndrome
(MERS), pneumonia (Bai et al., 2020). The symptoms are fever, cough, shortness of
breath, and breathing difficulties (Bai et al., 2020).
2. What clinical data from the chart is RELEVANT and needs to be trended because it is clinically
significant?
On admission body temperature is taken. This finding is significant because they are consistent
with the respiratory distress Covid 19.
3. What nursing priority captures the “essence” of your patient’s current status and will guide your
plan of care?
Risk of disease contamination
Risk on need of ventilators to support breathing difficulties.

Impaired gas exchange R/T fluid buildup and bacterial growth in lungs,
Risk for deficient fluid volume R/T insensible fluid loss, excess secretions and recent vomiting
Risk for (spread of) infection R/T current bacterial growth in the lungs (Bai et al., 2020).

4. What nursing interventions will you initiate based on this priority and what are the desired
outcomes?
Quarantine the person with disease to avoid from spreading the disease to other individuals.
Monitor breathing systems/air wave of the patient. As such, patient should maintain an airway
clear of secretions to allow for better gas exchange and decrease her work of breathing
Administer O2 as needed to keep saturation above 92%. The desired outcome is that the patient
will be able to wean off of O2 to achieve this level
Prescribe treatment using CDC guidelines include antibiotics, breathing treatments, prednisone,
O2, PRN analgesic and antipyretic. The objective is to eliminate the respiratory infection, and
allow her comfort and adequate oxygenation while this happens (Bai et al., 2020)
Monitor hydration status, vitals and physical assessment findings for dehydration and worsening
infection. The desired outcome is that the patient will remain free of these complications

priority?
Respiratory, immune system and reaction to antibiotic treatment. Circulatory system for
dehydration risk
