Relevant Background Information:
Coders review patient records, identify what information they need to abstract from the documentation,
and then assign the correct code(s) to that information. This is a critical skill that you need to develop
before you begin working as a medical coder.
Assignment Purpose:
This assignment asks you to review scenarios and assign the correct CPT code using your CPT code
manual and/or Nuance virtual encoder.
Assignment Directions:
Imagine that you are a coding supervisor and are creating a CCS-P preparation review test for your
coding staff. You have created some practice scenarios to test their CPT classification coding knowledge
to help them prepare for their exam. You have created some scenarios for your staff to code and now
need to create an answer key.
Follow the steps below to complete this assignment:
1. Review the scenarios in the chart below. For each scenario, identify which medical terms(s) or
phrase(s) you will use to code the scenario.
2. Use your CPT code book and/or Nuance virtual encoder to assign the correct CPT code to the
scenarios in the attached chart. Type your answers directly into the chart.
3. Review the attached rubric to determine how your work will be graded.
4. Submit your work by the designated due date.
