Physical assessment
Objective findings
The client is a twenty-three-year-old woman who works in a construction company that
manufactures heavy metals. She is very alert, conscious, and answers all questions properly. Her
skin is white with even tone and no visible lesion. She has a gloomy face that looks distressed.
Her height is five feet, three inches. She weighs one hundred and sixty pounds, which is
somehow overweight concerning her height. The body parts are correspondingly bilateral and in
a similar proportion. She seems to have difficulty while standing, thus made it easier to examine
her from a sitting position. She is unable to place her foot on the ground comfortably. The
movement of all joints is correct, smooth, and corresponding apart from her ankle, which is
inflexibly swollen and during movement. The lady maintains eye contact in the course of
communication but makes certain facial expressions such as occasional frown and whimpers,
especially during the examination of her ankle. Otherwise, she is cooperative and interacts
pleasantly. She is well nourished, appears clean, and appropriately groomed.
Subjective findings
The patient has no history of any skin disease, no allergies, eczema, or psoriasis. She has
no observable rash or lesions nor any signs of hair loss. She appears healthy since a pink color
returns in her nails in less than two seconds after a force is detached. The skin around her ankle
is red in color and visible irritation on the swollen area. No bruising observed.
The patient offered a two-day history of ankle pain and swelling. She also testifies of pain
as 5/10 with piercing aches when attempting to walk. When she rests and presses ice on the
