From the discussion board, it is evident that heart failure is a condition that is
characterized by blood pumping function of the organ is jeopardized, which contributes to the
under-perfusion of the tissues. The condition may result from several underlying conditions
including ischemic heart disease, congenital heart disease, hypertension, as well as valvular heart
disease (Roger, 2013). Heart disease may result into progressive physical as well as functional
deterioration of the organ resulting into shortness of breath, weight gain, tiredness, dizziness, as
well as swelling of parts of the body, such as ankles and abdomen.
Considering the case of the 78-year-old adult, it is evident that the symptoms of heart
disease are not easy to recognize by the patients and healthcare providers since the underlying
causes of the disease are varied. Besides, patients may experience complicated transitions
between care healthcare personnel, as well as healthcare facilities. Patients with heart failure are
indeed most likely to survive, especially when treated and followed up by healthcare specialists
(Roger, 2013). On the other hand, most patients diagnosed with heart disease may also
experience other conditions, which may need attention from different healthcare personnel. As
mentioned, heart failure may indeed exhibit certain symptoms that may require early
intervention, especially before exacerbation. Some common symptoms comprise of lower
extremity oedema, shortness of breath, as well as swelling of the ankles. The other symptoms
include an increase in weight.
Overall, patients diagnosed with heart failure need to be educated, since it is an important
