Medication Administration Errors
Medication administration errors present a huge risk to many patients in health care
organizations and present a risk to overall quality. Errors may occur at any time between the
point of prescription and medication administration. Some of the common types of medication
administration errors include administering to the wrong patient, incorrect route of
administration, wrong rate of administration, and overdosing the patient. These errors are
avoidable through quality improvement involving nurses and other health care stakeholders.
Administration of medication to the wrong patient is a common error linked to patient
identification. This paper focuses on reduction of errors of wrong patient administration through
the collaboration or nurses and other stakeholders in care coordination.
Factors Leading to Wrong Patient Medication Administration
Medication errors are caused by either human or system and organizational factors.
Human errors related to nursing may be caused by various factors which are also related to
wrong patient medication administration. Common nursing factors associated to medication
administration errors include workload and distractions (Tariq et al., 2020). The current
healthcare industry is undergoing a shortage of staff and nurses are the largest workforce in the
industry. With strain on nurses increasing, the serving nurse may have a high workload which
reduces their attention to detail. For instance, they may confuse names and patient identities
hence administering medication to the wrong patients. Being busy also leads to distractions
because nurses may have to multitask and switch from one patient to another, back and forth.
These distractions allow errors to creep in and wrong patient administration of medication is one
of the common errors.
