Nausea and vomiting- is a common side effect in early pregnancy due to the rise in human
chorionic gonadotropin (HCG) hormones when the egg is fertilized.
lower back pain- can be related to changes in the muscle skeleton that happens when the fetus is
growing, a history of lower back pain can worsen during pregnancy.
Right calf pain- expecting moms may experience leg cramps during the day or in the evening.
Leg cramps aren't usually a health risk but can be caused by weight gain and changes in
circulation during pregnancy. It is important to educate the client on signs of DVT.
Urinary frequency- can be caused by the growing fetus pushing up against the bladder.
Frequent urination can also be caused by increased maternal blood volume which can lead to
increased filtration.
2.) Identify which of the four symptoms on the list is the priority and explain why.
Nausea and vomiting is a priority finding in this patient who is 16 weeks gestation, having
nausea and vomiting past 12 weeks gestation can cause more serious problems like Hyperemesis
gravidarum and dehydration, weight loss, and lack of vitamins (Gregory et al., 2018). The patient
should try to eat smaller meals throughout the day to help with nausea and vomiting.

Good Job Charmagne! I like how you mentioned the lack of nutrients, electrolyte imbalance, and
dehydration that would be caused by the mother having nausea and vomiting. I would also
include to educated the mother on choosing foods carefully, and select foods that are high in
protein, low in fat, easy to digest, and avoid greasy, spicy food. Also educating the mother to
drink a lot of fluids to help with dehydration would be good. Nausea and vomiting past 12 weeks
could mean the patient is experiencing hyperemesis gravidarum that can lead the mother to being
hospitalized. Good job again Charmagne!

The nurse needs to assess the patient's right calf to check for warmth, redness, and swelling. The
patient could possibly have a deep vein thrombosis DVT which is a blood clot that can be
dislodged. The DVT can dislodge and travel “embolism”. This can travel to the patient lungs and
cause a Pulmonary embolism.
