Pathophysiology
Diarrhea occurs when the
bowel movements are more
loose and watery than normal.
The intestines push stool into
the rectum before water is
reabsorbed in the bloodstream,
making one to experience
diarrhea. Diarrhea also occurs
when the bowel experiences
inflammation leading to
excessive fluid leakage into the
stool (Sheikh et al., 2018).
Bowel obstruction occurs
when the bowel or intestines
experience blockage making
digestive secretions, ingested
food, and fluid and gas
accumulate above the blockage
(Reddy & Cappell, 2017). The
bowel becomes congested and
edematous leading to
strangulating obstruction.
Deterioration or disintegration
of the tissues of the anal
cushions leads to Hemorrhoids
(Idrees et al., 2019). The anal
cushions are therefore
displaced downwards, leading
to venous dilatation. This is
referred to as the sliding anal
canal lining theory.
Etiology
Diarrhea is mainly caused by
viral, bacterial, and parasitic
infections. Some of the
common viruses include
norovirus and Viral
gastroenteritis. Salmonella and
Escherichia coli are common
bacteria. Eating or drinking
contaminated food or water
cause traveler’ diarrhea
(Sheikh et al., 2018).
One of the common causes of
bowel obstruction is intra-
abdominal adhesions, leading
to about 75% of all cases
(Reddy & Cappell, 2017).
Hernias, malignancy, Crohn
disease, and volculus are also
other common causes.
Mediators or enzymes
contribute to the degradation of
the anal cushions and
supporting tissues. One of
them is the matrix
metalloproteinase (MMP)
(Idrees et al., 2019).

another cause of hemorrhoids,
mainly when Stomal Vascular
Endothelial factors (VEGF)
