Decision #1: Differential Diagnosis
Several clinical features lead to the conclusion that Mr. Charles is suffering from Major
Neurocognitive Disorder with Lewy Bodies. His daughter reported of his bad memory and the
high tendency to forget things. Mr. Wingate is reported to have a good memory during the hours
of the morning as compared to the evening. Mr. Charles also reported experiencing nightmares
and poor sleep for quite some time. He also reported of having fluctuation in his energy levels
throughout the day.
The differential diagnosis include: Parkinson disease, Major Neurocognitive Disorder as a result
of Alzheimer’s disease, REM Sleep Disorder.

Of the three alternatives, I chose Major Neurocognitive Disorder with Lewy Bodies
Why this decision was selected
I made this selection since at the age of 76, Mr. Wingate has forgetfulness that has been present
for some time, fluctuations in energy levels, and hallucinations at night which are symptoms
consistently related with Major Neurocognitive Disorder with Lewy Bodies. The patients found
with this disorder exhibit a remarkable dysfunction of the sleep and motor, as well as
neurocognitive and neuropsychiatric symptoms (How et al., 2018). Because of the following
signs, the diagnosis of Mr. Wingate, after using the DSM-5 criteria, meets Major Neurocognitive
Disorder with Lewy Bodies:
Clearly developed hallucinations that tend to be repeated on occasion. The patient can also act
out their dreams when sleeping. Moreover, the patient also has sleep difficulties and memory
loss. The cognitive thinking of the patient is also fluctuating and unpredictable.
What I was hoping to achieve with this Decision
My decision was aimed at an accurate diagnosis of the patient. To achieve this, the clinical
features presented by the patient must be thoroughly assessed. In order to follow the plan easily
