Like you, I chose diagnostic imaging to discuss in how risk management can address this high risk area. Unlike your post, I did not emphasize or mention error disclosure as being one of the areas of complication. Error disclose is definitely one of the most common areas for issues. In reponse to disclose errors you mentioned, "The risk management team should establish a culture in the diagnostic imaging department whereby caregivers disclose errors to each other to promote transparency and decrease exposure liability" - Although I do agree with full transparency within an organization; what are your thoughts on establishing this kind of informed consent with patients prior to their diagnostic testing?
